目的:通过对40例All-on-4种植即刻修复病例临床总结,探讨All-on-4种植技术的优势、成功的关键以及并发症的防范。方法:2008年5月-2012年11月共40例患者(男32例,女8例)接受了All-on-4种植即刻修复。40例(上无牙颌23例,下无牙颌9例,双颌无牙颌8例)共植入192枚种植体,在种植体植入当天完成即刻修复,3-4个月后完成永久修复。即刻修复后观察种植体边缘骨吸收情况、追踪种植体和修复体的存留率。结果:192枚种植体中,5枚于植入后6-16周即刻负重期间脱落,4枚于永久修复后2周脱落。有一例病人因咬合、清洁、吸烟等因素致边缘骨吸收明显。其余179枚种植体至最后一次复查临床情况稳定。上颌种植体存留率93%,下颌种植体存留率99%。修复体存留率100%。边缘骨吸收平均0.7mm。讨论:All-on-4种植即刻修复技术与常规无牙颌种植技术有何不同?如何保障上颌All-on-4种植即刻修复的成功率?结论:All-on-4种植即刻修复技术应用于无牙颌重建高效、可靠。但技术要求较高,特别是上颌All-on-4种植即刻修复要求种植体对侧向力有足够的抵抗能力。
Objective: To investigate the advantages of All-on-4 implantation, the key to success and the prevention of complications through the clinical summary of 40 cases of immediate all-on-4 implantation. METHODS: From May 2008 to November 2012, 40 patients (32 males and 8 females) received immediate repair of All-on-4 implants. A total of 192 implants were implanted in 40 cases (23 cases with maxillary edentulous, 9 cases with edentulous edentulous, 8 cases with edentulous edentulous) completed at the implant site and completed immediately after 3-4 months Permanent repair. Immediately after restoration, the marginal bone resorption was observed and the survival rate of implants and prostheses was tracked. Results: Of the 192 implants, 5 were shed at 6-16 weeks immediately after implantation and 4 were shed 2 weeks after permanent repair. One case of patients with occlusal, clean, smoking and other factors caused by the edge of the bone absorption significantly. The remaining 179 implants to the last review of clinical stability. Maxillary implant survival rate of 93%, mandibular implant survival rate of 99%. Prosthesis survival rate of 100%. Edge bone absorption average 0.7mm. DISCUSSION: What is the difference between All-on-4 implants and conventional edentulous implants? How to ensure the success of immediate implants in maxillary All-on-4 implants? Conclusion: All-on-4 implants are used in Edentulous reconstruction efficient and reliable. However, the higher technical requirements, especially the maxillary All-on-4 implants, implants require lateral implants have sufficient resistance.
