目的:通过观察胃癌、癌前病变中细胞凋亡和凋亡调控基因bcl-2、c-myc的表达,探讨细胞凋亡与调控基因在胃粘膜组织恶性转化中的作用。方法:应用原位末端标记法(TUNEL)、免疫组织化学染色及逆转录PCR(RT-PCR)法,观察76例胃癌,42例肠上皮化生和34例异型增生胃粘膜活检组织细胞凋亡和bcl-2、c-myc的表达,并以20例慢性浅表性胃炎作对照。结果:胃癌细胞凋亡指数明显低于肠化和异型增生(P<0.05)及慢性浅表性胃炎(P<0.01);肠化生、异型增生细胞凋亡指数分别低于慢性浅表性胃炎(P<0.05)。胃癌和异型增生bcl2阳性表达率明显高于慢性浅表性胃炎(P<0.01),肠化生亦高于慢性浅表性胃炎(P<0.05);bcl-2阳性胃癌细胞凋亡指数低于bcl-2阴性胃癌(P<0.05)。胃癌、肠化生和异型增生cmyc阳性表达率分别高于浅表性胃炎(P<0.01);c-myc阳性胃癌、异型增生的细胞凋亡指数分别高于阴性胃癌(P<0.05)。结论:细胞凋亡调控基因bcl-2、c-myc失调在胃组织恶性病变转化过程中起重要作用,bcl-2可抑制细胞凋亡,而cyc则对胃癌细胞凋亡具有促进作用。
OBJECTIVE: To investigate the role of apoptosis and regulation genes in the malignant transformation of gastric mucosa by observing the apoptosis and expression of apoptosis-regulating genes bcl-2 and c-myc in gastric cancer and precancerous lesions. METHODS: Apoptosis of gastric mucosa was examined by in situ end labeling (TUNEL), immunohistochemistry, and reverse transcription-PCR (RT-PCR) in 76 cases of gastric cancer, 42 cases of intestinal metaplasia, and 34 cases of dysplasia. And expression of bcl-2, c-myc, and 20 cases of chronic superficial gastritis as a control. Results: The apoptosis index of gastric cancer cells was significantly lower than that of intestinal metaplasia and dysplasia (P<0.05) and chronic superficial gastritis (P<0.01). The apoptotic index of intestinal metaplasia and dysplasia was lower than that of chronic superficial gastritis, respectively. (P<0.05). The positive rate of bcl2 expression in gastric cancer and dysplasia was significantly higher than that in chronic superficial gastritis (P<0.01). Intestinal metaplasia was also higher than chronic superficial gastritis (P<0.05). The apoptosis index of bcl-2 positive gastric cancer cells was lower than that of gastric cancer. Bcl-2 negative gastric cancer (P<0.05). The positive expression rate of cmyc in gastric cancer, intestinal metaplasia and dysplasia was higher than that in superficial gastritis (P<0.01). The apoptotic index of c-myc positive gastric carcinoma and dysplasia was higher than that of negative gastric carcinoma (P<0.05). Conclusion: The dysregulation of apoptosis-regulating genes bcl-2 and c-myc plays an important role in the transformation of gastric malignant lesions. bcl-2 can inhibit apoptosis, while cyc can promote the apoptosis of gastric cancer cells.
