目的:探究细胞因子TNF-α、IL-6和IL-8在白血病异基因造血干细胞移植后的变化和病情及预后的关系。方法:用放射免疫分析检测了白血病异基因造血干细胞移植患者(48例)、正常健康人(30例)血清TNF-α、IL-6和IL-8水平,对移植后发生或未发生急性移植物抗宿主病(aGVHD)及发生或未发生慢性移植物抗宿主病(cGVHD)进行了比较,并对预后也进行比较。结果:急、慢性GVHD组与未发生急、慢性GVHD组患者的血清TNF-α、IL-6及IL-8水平均高于正常对照组(P<0.05,除未发生cGVHD组的TNF-α浓度低外);急、慢性GVHD组与未发生急、慢性GVHD组比血清TNF-α、IL-6及IL-8水平均升高(P<0.05);移植后死亡组生存时间≤6月及>6月(除>6月的IL-8水平外)各组TNF、IL-6与IL-8水平均高于同期的存活组(P<0.05)。结论:异基因造血干细胞移植术后动态监测TNF、IL-6、IL-8水平变化有助于对发生aGVHD或cGVHD的可能提供另一种检测方法,并对判断愈后有一定的临床意义。
Objective: To investigate the relationship between the changes of cytokines TNF-α, IL-6 and IL-8 after leukemia allogeneic hematopoietic stem cell transplantation and their prognosis. Methods: Serum levels of TNF-α, IL-6 and IL-8 in patients with leukemia allogeneic hematopoietic stem cell transplantation (48 cases) and normal healthy people (30 cases) were detected by radioimmunoassay. (AGVHD) with or without chronic graft-versus-host disease (cGVHD) were compared and the prognosis was also compared. Results: Serum levels of TNF-α, IL-6 and IL-8 in patients with acute and chronic GVHD and those without GVHD were significantly higher than those in normal controls (P <0.05, except TNF-α The levels of TNF-α, IL-6 and IL-8 in serum of acute and chronic GVHD group were significantly higher than those of non-acute and chronic GVHD group (P <0.05); the survival rate of death group after transplantation was less than 6 months And> 6 months (except IL-8 level> 6 months). The levels of TNF, IL-6 and IL-8 in all groups were higher than those in the same period (P <0.05). CONCLUSION: The dynamic changes of TNF, IL-6 and IL-8 levels after allogeneic hematopoietic stem cell transplantation are helpful to provide another detection method for aGVHD or cGVHD, and have some clinical significance in judging prognosis.
