目的探讨容积再现(VR)和最大强度投影(MIP)两种螺旋CT三维重组技术在评价儿童骨骼异常中的应用价值。方法收集2014年3月-6月来四川大学华西第二医院检查的患儿中,螺旋CT横断层图像及多平面重组诊断为骨骼异常的患儿18例,对其原始数据进行VR和MIP。由2名放射科医师独立评价所有三维图像(1~4分),4分者能完全诊断病变。对所有评分进行Wilcoxon秩和检验,分别计算Kappa值。结果 VR和MIP的图像评分分别为(3.28±0.96)分和(2.89±0.90)分,差异无统计学意义(Z=-1.732,P=0.125)。VR和MIP的Kappa值分别为0.730和0.593,具有高度和中度一致性。结论 VR和MIP在显示空间立体结构方面有独到的优势,能辅助CT二维图像,更加全面、综合地评估儿童骨骼异常。
Objective To investigate the value of volumetric reconstruction (VR) and maximum intensity projection (MIP) two-dimensional spiral CT three-dimensional reconstruction in the evaluation of skeletal abnormalities in children. Methods From March to June 2014, 18 children with skeletal abnormalities diagnosed by spiral CT scan and multiplanar reconstruction in children examined at West China Second Hospital of Sichuan University from March 2014 to June were retrospectively analyzed. Their original data were collected for VR and MIP. Two radiologists independently evaluated all three-dimensional images (1 to 4) and 4 were able to fully diagnose the lesion. All scores were Wilcoxon rank sum test, calculated Kappa value. Results The image scores of VR and MIP were (3.28 ± 0.96) points and (2.89 ± 0.90) points respectively, with no significant difference (Z = -1.732, P = 0.125). The Kappa values for VR and MIP were 0.730 and 0.593, respectively, with high and moderate agreement. Conclusion VR and MIP have unique advantages in displaying the spatial structure of the space. They can assist the two-dimensional CT images and evaluate the skeletal abnormalities in children more comprehensively and comprehensively.
