目的急性脑梗死患者血清尿酸水平与脑微出血(cerebral microbleed,CMB)的相关性研究。方法连续性收录作者医院2012-01/2015-01月收治的急性脑梗死患者220例,根据磁共振成像(magnetic resonance imaging,MRI)结果分为CMB组(n=90)和非CMB组(n=130)。分析比较入院当天和出院时血尿酸水平与CMB的相关性。结果入院时CMB组患者尿酸水平明显低于非CMB患者,2组比较差异具有统计学意义(P<0.05),CMB重度亚组患者明显高于轻度亚组患者,2组比较差异具有统计学意义(P<0.05)。对于CMB诊断价值方面,可见入院时尿酸的曲线下面积(area under the curve,AUC)为0.802,cut-off值为284.22,敏感性为80.30%、特异性为75.71%。出院时尿酸的AUC为0.713,cut-off值为260.31,敏感性为79.14%,特异性为82.22%。入院时尿酸(r=0.722,P<0.05)和出院时尿酸(r=0.682,P<0.05)水平与CMB均呈正相关。结论急性脑梗死患者血尿酸水平与CMB密切相关,是诊断CMB的潜在生物学标记物。
Objective To study the correlation between serum uric acid level and cerebral microbleed (CMB) in patients with acute cerebral infarction. Methods The data of 220 consecutive patients with acute cerebral infarction who were admitted to the hospital from January 2012 to January 2015 were divided into CMB group (n = 90) and non-CMB group (n = 90) according to the magnetic resonance imaging (MRI) = 130). Analysis and comparison of serum uric acid levels and CMB on admission day and discharge correlation. Results The level of uric acid in CMB group was significantly lower than that in non-CMB group on admission. There was significant difference between the two groups (P <0.05). The CMB severe subgroup was significantly higher than that of mild subgroup. The difference between the two groups was statistically significant Significance (P <0.05). For the diagnostic value of CMB, it can be seen that the area under the curve (AUC) of admission of uric acid was 0.802, the cut-off value was 284.22, the sensitivity was 80.30% and the specificity was 75.71%. The uric acid AUC at discharge was 0.713, the cut-off value was 260.31, the sensitivity was 79.14% and the specificity was 82.22%. Uric acid (r = 0.722, P <0.05) at hospital admission and uric acid at discharge (r = 0.682, P <0.05) were positively correlated with CMB. Conclusion Serum uric acid level is closely related to CMB in patients with acute cerebral infarction and is a potential biomarker for the diagnosis of CMB.
