目的了解武汉市江岸区居民脑卒中死亡特征,为相关部门制定防控政策提供依据。方法分析江岸区2003—2014年居民脑卒中死亡登记资料,对脑卒中死亡的粗死亡率、标化死亡率和死因构成比的变化特点进行描述性分析。结果江岸区居民脑卒中死因顺位由2003年的第1位(死因构成比为27.33%)下降为2014年的第2位(20.40%)。87.81%的脑卒中死亡发生在60岁及以上人群,其中60.63%的死亡发生在75岁及以上人群。2003—2014年各年龄组人群脑卒中死亡率均有较明显下降,其中60岁~和75岁以上人群死亡率分别由576.86/10万和2 589.20/10万下降为180.58/10万和1 357.23/10万,年均下降速度分别为11.14%和6.05%。男性脑卒中死亡率高于女性,2008—2014年男性和女性粗死亡率均呈明显下降趋势,年均下降速度男女性分别为6.34%和6.01%。2013—2014年缺血性脑卒中占脑卒中死亡的60.66%~73.15%。结论江岸区2003—2014年居民脑卒中死亡整体呈现一定下降趋势,但脑卒中仍是主要死因之一。应有针对性地开展重点防控和早期筛查工作。
Objective To understand the characteristics of stroke death among residents in Jiang’an District of Wuhan City and to provide basis for relevant departments to make prevention and control policies. Methods The data of stroke deaths among residents in Jiangan District from 2003 to 2014 were analyzed to describe the changes of crude death rate, standardized mortality rate and cause of death in stroke patients. Results The rank of the cause of death from stroke in Jiang’an District dropped from No. 1 in 2003 (the cause of death constituent ratio was 27.33%) to No. 2 in 2014 (20.40%). 87.81% of stroke deaths occurred in people aged 60 and above, of which 60.63% of deaths occurred in people aged 75 and over. In 2003-2014, the death rates of stroke in all age groups were significantly decreased, among which the death rates of 60-year-olds and over 75-year-old people dropped from 576.86 / 100000 and 2589.20 / 100000 to 180.58 / 100000 and 1 357.23 / 100,000, with an average annual decline rate of 11.14% and 6.05%. The mortality rate of stroke in men is higher than that in women. The crude death rate of males and females in 2008-2014 showed a declining trend with an average annual rate of decline of 6.34% and 6.01% for males and females respectively. 2013-2014 ischemic stroke accounted for 60.66% ~ 73.15% of stroke deaths. Conclusion The overall death rate of residents in Jiang’an District from 2003 to 2014 shows a certain downward trend, but stroke is still one of the major causes of death. Should focus on prevention and control and early screening should be carried out.
