血清免疫球蛋白游离轻链κ和λ水平测定是多发性骨髓瘤和相关浆细胞疾病诊断和处理中的重要工具。血清免疫球蛋白游离轻链分析与血清蛋白电泳和免疫固定电泳联合可明显提高浆细胞疾病诊断的敏感性,并在除轻链淀粉样变性外的其他疾病中替代24h尿液检查。基础血清免疫球蛋白游离轻链可预测各种浆细胞病的预后。对轻链淀粉样变性、寡分泌型和非分泌型骨髓瘤,可通过血清免疫球蛋白游离轻链分析进行定量监测。对轻链淀粉样变性,连续血清免疫球蛋白游离轻链检测优于蛋白电泳和免疫固定电泳。
Serum immunoglobulin free light chain κ and λ levels are important tools in the diagnosis and treatment of multiple myeloma and related plasma cell disorders. Serum immunoglobulin free light chain analysis combined with serum protein electrophoresis and immunostaining electrophoresis can significantly improve the sensitivity of plasma cell disease diagnosis and replace24h urine examination in other diseases except light chain amyloidosis. The basal serum immunoglobulin free light chain predicts the prognosis of various plasma cell disorders. For light-chain amyloidosis, oligoclonal and non-secreting myeloma, quantitative immunoglobulin free light chain analysis can be used for quantitative monitoring. For light chain amyloidosis, continuous serum immunoglobulin free light chain detection is superior to protein electrophoresis and immunofixation electrophoresis.
