目的调查云南省流行性乙型脑炎(乙脑)患者感染乙脑病毒(JEV)基因型状况。方法在云南省西双版纳州医院和大理州医院采集脑炎患者急性期血清标本20份和脑脊液标本11份,用RT-PCR法检测JEV核酸,阳性者进行JEV-PreM/C区基因序列测定,用核苷酸同源性和系统进化分析进行基因型鉴定。结果用RT-PCR法从11例脑炎患者脑脊液标本中检测到JEV核酸阳性3份(JB114、JB135和DLN24),而20例脑炎患者血清标本均为阴性。3株病毒的PreM/C区核苷酸序列同源性分析显示,JB114、JB135与JEV基因Ⅰ型代表株同源性高达98.3%和99.2%,而与Ⅱ、Ⅲ、Ⅳ型仅为82.5%~84.6%;DLN24与基因Ⅲ型代表株同源性为94.6%,而与基因Ⅰ、Ⅱ、Ⅳ型分别为85.0%、82.5%和80.4%。进化树分析显示,JB114和JB135位于基因Ⅰ型分支中,DLN24位于基因Ⅲ型分支上。结果表明,来自西双版纳的JB114和JB135病毒株属于基因Ⅰ型,来自大理的DLN24病毒株为基因Ⅲ型。结论云南省乙脑患者中分别存在基因Ⅰ型和Ⅲ型JEV感染的病例,表明云南省存在这两个基因型病毒的共同流行。
Objective To investigate the status of JEV genotypes in patients with Japanese encephalitis (JE) in Yunnan Province. Methods Twenty encephalitis samples and 11 CSF samples were collected from Xishuangbanna Hospital and Dali Hospital in Yunnan Province. JEV-PreM / C gene sequences were detected by RT-PCR and positive for JEV-PreM / Nucleotide homology and phylogenetic analysis. Results Three JEV nucleic acid positive samples (JB114, JB135 and DLN24) were detected in cerebrospinal fluid of 11 patients with encephalitis by RT-PCR, while the serum samples of 20 encephalitis patients were negative. The nucleotide sequence homology analysis of PreM / C region showed that the homology of JB114, JB135 and JEV genotype Ⅰ strains was as high as 98.3% and 99.2%, while it was only 82.5% with type Ⅱ, Ⅲ and Ⅳ, ~ 84.6% respectively. The homology between DLN24 and genotype Ⅲ was 94.6%, while it was 85.0%, 82.5% and 80.4% with genotypes Ⅰ, Ⅱ and Ⅳ respectively. Phylogenetic tree analysis showed that JB114 and JB135 are located in the gene type Ⅰ branch and DLN24 is located in the gene type Ⅲ branch. The results showed that JB114 and JB135 strains from Xishuangbanna belonged to genotype Ⅰ, and DLN24 from Dali was genotype Ⅲ. Conclusions There are cases of genotypes I and III of JEV infection in patients with JE in Yunnan Province, indicating the common epidemic of these two genotypes in Yunnan Province.
