目的了解北京市部分地区的传染病患者在社区卫生服务机构的就诊情况,探讨社区卫生服务机构在传染病防治工作中存在的问题及其改进措施。方法于2009年10月和2010年6月两个时间点,对在北京市四个城区(东城区、西城区、朝阳区、顺义区)所属社区卫生服务机构就诊的传染病患者进行问卷调查。结果在社区卫生服务机构就诊的传染病患者,以慢性疾患如肝炎、结核为主;急性传染病以腹泻为主。患者在病情危重的情况下首选综合/专科医院就诊;但在一般情况下,如果社区卫生服务机构和综合/专科医院合作关系良好,愿意在社区卫生服务机构就诊并留存病案的患者比例明显增加。患者希望通过就诊时医生的讲解和参加系统的传染病知识讲座来获取相关知识;建议社区卫生服务机构提高诊疗水平、与综合/专科医院建立良好的合作关系。结论社区卫生服务机构和综合/专科医院建立良好的合作关系,可以形成有利于患者、社区卫生服务机构以及综合/专科医院的三方互利的模式。
Objective To understand the visiting status of infectious disease patients in community health service institutions in some areas of Beijing and to explore the problems existing in the prevention and control of infectious disease in community health service institutions and the improvement measures. Methods A questionnaire survey was conducted on infectious disease patients in community health service institutions affiliated to four urban districts of Beijing (Dongcheng District, Xicheng District, Chaoyang District and Shunyi District) at two time points from October 2009 to June 2010. Results Among the infectious disease patients attending community health service, chronic diseases such as hepatitis and tuberculosis were the major ones; acute infectious diseases were mainly diarrhea. In general, if the community health service and general hospital / specialist hospital have a good working relationship, the proportion of patients willing to visit the community health service and keep the medical record is significantly increased. Patients hope to get relevant knowledge through doctor’s explanation and attending systematic infectious disease knowledge lectures at the time of treatment; recommend community health service institutions to improve diagnosis and treatment level and establish good cooperative relations with comprehensive / specialized hospitals. Conclusion The establishment of good cooperative relations between CHS and general hospitals / specialized hospitals can create a model of tripartite reciprocity that benefits patients, community health services and general hospitals.
