目的:了解上海市虹口区人乳头状瘤病毒(HPV)亚型分布,探讨不同HPV亚型感染与宫颈病变程度的关系。方法:采用核酸分子快速导流杂交技术,对538例因细胞学结果异常或本人意愿行宫颈分泌物HPV-DNA亚型检测,对其中228例HPV阳性患者阴道镜下定位宫颈组织活检和(或)ECC、LEEP术,以最高级别的病理结果为最终病理学诊断。根据人乳头状瘤病毒亚型分布特点,分析其与宫颈病变的关系。结果:228例HPV阳性病例中,检测到21种亚型中的18种,未检测到HPV-35、42、51三种亚型。感染频率前6位依次为:HPV-16(29.82%)、58(19.74%)、52(17.54%)、33(9.21%)、53(9.21%)和11(5.26%)。样本中单型感染184例,多型感染44例。在宫颈高度病变组中HPV亚型感染依次为HPV-16、52、58、33、31/45/53、18/CP8304,但仅HPV-16与宫颈病变程度明显相关,与正常或炎症组、低度病变组比较,差异有统计学意义(P<0.001)。在多型感染中,高度病变组与低度病变组比较具有统计学意义(P<0.05)。结论:高危型HPV-16是上海市虹口区主要的感染亚型和致病型,混合感染在一定程度上促进了疾病的发展。
Objective: To understand the distribution of human papillomavirus (HPV) in Hongkou District of Shanghai and to explore the relationship between different HPV subtypes and cervical lesions. Conclusion: High-risk HPV-16 is the major infectious subtype and pathogenic type in Hongkou District, Shanghai. The mixed infection can promote the development of the disease to a certain extent.
