目的临床上各种病因所致缓慢性心律失常如病态窦房结综合征、房室传导阻滞等伴明显血流动力学障碍及心肺复苏的抢救、外科手术时的保护等情况下需紧急安置临时起搏器以维持基本心率保持血流动力学稳定,本研究旨在讨论床旁心电监护下非X线植入临时起搏器的安全性及有效性。方法 10例三度AVB及8例病态窦房结综合征患者,在床旁通过穿刺右锁骨下静脉或右颈内静脉,通过观察起搏信号、QRS波群形态及测定阈值将普通电极导管送入右心室。结果 18例患者穿刺均获成功并成功起博心脏。结论经右锁骨下静脉或右颈内静脉穿刺床旁临时起搏术创伤小、方便快速、安全有效。

Methods Ten patients with third degree AVB and eight patients with sick sinus syndrome were punctured through the right subclavian vein or right internal jugular vein at the bedside to observe the pacing signal, Into the right ventricle. Results All the 18 patients were successfully punctured and pacing the heart successfully. Conclusion Proximal subclavian vein or right internal jugular vein puncture temporary bedside trauma is small, convenient and fast, safe and effective.
