目的探讨急性脑卒中伴昏迷的老年患者单次鼻饲量与吸入性肺炎发生率的关系。方法选择2009年6月至2011年3月我科收治的218例老年急性脑卒中、发病1周以内伴昏迷并予鼻饲的患者,随机分为干预组和对照组;在鼻饲置管方法、鼻饲体位、鼻饲液成分、日鼻饲总量相同的前提下,对照组按常规方法进行鼻饲(单次鼻饲营养液250ml,每日4次;水250ml每日4次和营养液交替注入;共计8次);干预组按照实验方法鼻饲(单次鼻饲营养液200ml,每日5次注入;水200ml每日5次和营养液交替注入;共计10次),其他护理方案相同,观察两组患者30d内肺炎发生率。结果对照组112例中,肺炎发生率为20.5%(23例);干预组106例中,肺炎发生率为9.4%(10例),两组间肺炎发生率的差异有统计学意义(P<0.05)。两组间营养状况指标(血清总蛋白、血清白蛋白、血红蛋白)、免疫力指标(淋巴细胞计数)的差异无统计学意义(P>0.05)。结论急性脑卒中伴昏迷并予鼻饲的老年患者,采用较小的单次鼻饲量缓慢注入有助于减少吸入性肺炎的发生率。
Objective To investigate the relationship between single nasal feeding and the incidence of aspiration pneumonia in elderly patients with acute stroke and coma. Methods A total of 218 elderly patients with acute stroke admitted to our department from June 2009 to March 2011 were randomly divided into intervention group and control group. There was no significant difference in nutritional status (serum total protein, serum albumin, hemoglobin) and immunity index (lymphocyte count) between the two groups (P> 0.05). Conclusion In elderly patients with acute stroke and coma and nasal feeding, the slow single injection of nasal feeding may help to reduce the incidence of aspiration pneumonia.
