目的 观察联合应用前方牵引和上颌快速扩弓治疗 10～ 14岁骨性安氏Ⅲ类错牙合患者的临床效果。方法 以 11例 10～ 14岁骨性Ⅲ类错牙合患者为观察组 ,3 2例替牙早期骨性Ⅲ类错牙合患者为对照组。两组均采用联合应用前方牵引和上颌快速扩弓装置治疗 ,平均治疗时间 6个月。治疗前后均拍摄头影侧位片 ,并采用一种新的头颅参照系统 ,对结果进行分析比较 ,评价治疗效果。结果 两组患者均发生上颌骨前移 ,下颌骨向下后旋转 ,向前生长受限。观察组前牙覆盖增加 5 .3mm ,骨性变化占 5 1% ,牙性变化占 4 9% ;其中 ,9例患者前牙达到正常覆牙合、覆盖关系 ,磨牙中性关系 ,2例患者前牙达到浅覆牙合、浅覆盖关系 ,后牙中性偏近中关系。结论 联合应用前方牵引和上颌快速扩弓治疗 10～ 14岁轻中度骨性反牙合患者有较好疗效 ,可减轻畸形程度 ,避免手术治疗 ,最大程度地改善患者的软硬组织关系
Objective To observe the clinical effect of combined application of anterior traction and maxillary rapid expansion in the treatment of patients with skeletal Class Ⅲ malocclusion of 10 ~ 14 years old. Methods Eleven patients with skeletal Class Ⅲ malocclusion aged from 10 to 14 were selected as the observation group, and 32 patients with early skeletal Class Ⅲ malocclusion were selected as the control group. Both groups were treated with the combination of anterior traction and maxillary rapid expansion device with an average treatment time of 6 months. Before and after treatment were shot cephalometric films, and the use of a new head reference system, the results were analyzed and compared to evaluate the treatment effect. The results of the two groups of patients with maxillary advancement occurred, mandibular downward rotation, limited forward growth. The coverage of the anterior teeth of the observation group increased by 5.3 mm, the change of bony changes was 51% and the change of the dentures was 49%. Among them, the anterior teeth reached the normal occlusion, the coverage and the molar relationship in 9 patients, and 2 patients The anterior teeth reach shallow overbite, shallow covering relationship, posterior teeth neutral near relationship. Conclusions The combined application of anterior traction and maxillary rapid expansion of 10 to 14 years old patients with mild to moderate bony anti-occlusion has a good effect, can reduce the degree of deformity, to avoid surgery, to maximize the patient's soft and hard tissue relationship
