目的探讨自制专科宣教视频在择期前列腺增生电切术围手术期老年患者宣教中的应用及影响,为提高患者的心理精神状态和预后生存质量提供依据。方法选取2014年1月至2016年1月浙江省衢州市人民医院收治的300例前列腺增生行电切术的老年患者为研究对象,随机分为对照组和视频宣教组,每组150例。对照组给予术前常规宣教;病例组则在常规宣教的基础上增加自制专科宣教视频,在宣教前1周、手术后1周内运用焦虑自评量表、抑郁自评量表及世界卫生组织生存质量测定量表分别对两组患者进行心理精神状态评估和生存质量评价,并统计住院患者的满意度、平均住院天数等。用SPSS 19.0软件进行t检验,χ~2检验和秩和检验。结果两组患者在宣教前焦虑自评量表、抑郁自评量表和世界卫生组织生存质量测定量表的评分比较,差异均无统计学意义(P>0.05)。宣教后视频宣教组的焦虑和抑郁评分分别为(32.5±6.8)和(31.2±6.3)分,明显低于对照组的(37.6±7.3)和(37.1±6.1)分,差异均有统计学意义(P<0.01)。宣教前两组间生存质量评分差异无统计学意义(P>0.05),宣教后视频宣教组的各项评分有显著提升,其中生理、心理、社会和环境4个领域得分[分别为(68.5±15.4)、(69.1±10.1)、(64.5±15.1)和(61.7±14.3)分]明显高于对照组[分别为(56.6±14.0)、(55.1±11.2)、(56.9±14.9)和(53.2±14.5)分],差异均有统计学意义(P<0.01)。视频宣教组满意度为96.7%,明显优于对照组(84.7%);住院天数为(6.7±3.2)d,少于对照组[(9.9±4.5)d],差异均有统计学意义(P<0.05,P<0.01)。结论运用自制专科宣教视频对择期前列腺增生电切术围手术期老年患者进行宣教,能有效降低患者的焦虑和抑郁状态,提高患者生活质量,值得推广。
Objective To explore the application and influence of home-made specialist mission video in the perioperation of elderly patients in elective prostatectomy and to provide basis for improving mental health status and prognosis of patients. Methods From March 2014 to January 2016, 300 elderly patients with benign prostatic hyperplasia (PRH) who underwent resection of the prostate in Quzhou People’s Hospital of Zhejiang Province were enrolled and randomly divided into control group and video mission group, 150 cases in each group. The control group was given preoperative routine education; the case group was based on the conventional mission to add self-made specialist mission video, 1 week before the mission, within 1 week after surgery using self-rating anxiety scale, self-rating depression scale and the World Health Organization The quality of life measurement scale was used to evaluate the mental state and quality of life of the two groups of patients respectively, and the satisfaction degree of inpatients and the average hospitalization days were calculated. SPSS 19.0 software t test, χ ~ 2 test and rank sum test. Results There was no significant difference in scores of self-rating anxiety scale, depression self-rating scale and WHO quality of life scale before the mission in both groups (P> 0.05). The scores of anxiety and depression in the mission video mission group were (32.5 ± 6.8) and (31.2 ± 6.3) points, respectively, which were significantly lower than those in the control group (37.6 ± 7.3) and (37.1 ± 6.1) (P <0.01). There was no significant difference in the quality of life between the two groups before the mission (P> 0.05). The score of the mission group increased significantly, among them, scores in the four fields of physiology, psychology, society and environment were 68.5 ± 15.4), (69.1 ± 10.1), (64.5 ± 15.1) and (61.7 ± 14.3) points were significantly higher than those in the control group (56.6 ± 14.0, 55.1 ± 11.2, 56.9 ± 14.9 and 53.2 ± 14.5) points, the differences were statistically significant (P <0.01). Video mission group satisfaction was 96.7%, significantly better than the control group (84.7%); hospital stay was (6.7 ± 3.2) d, less than the control group [(9.9 ± 4.5) d], the difference was statistically significant (P <0.05, P <0.01). Conclusions The use of self-made specialist mission video for elective prostate hyperplasia resection of elderly patients in the perioperative mission, can effectively reduce the patient’s anxiety and depression, improve patient quality of life, it is worth promoting.
