目的探究孕前肥胖对妊娠结局的影响。方法选取2012年1月-2014年12月在深圳市妇幼保健院进行产前检查并分娩的产妇2410例,其中正常组产妇1777例,比率为73.73%,超重组产妇498例,比率为20.66%,肥胖组产妇135例,比率为5.60%。按照其体重指数将其分为正常组、超重组及肥胖组,对其进行相应的统计学分析,探究产前肥胖对孕妇妊娠结局的影响情况。结果超重组和肥胖组产妇的妊娠高血压、糖尿病、胎膜早破、剖宫产率以及产后出血发生率均明显高于正常组,同时肥胖组也明显高于超重组,差异具有统计学意义(P<0.05),肥胖组的胎儿娩出超重比率明显高于正常组和超重组,超重组和肥胖组的早产率明显高于正常组,分娩呼吸窘迫的发生率明显低于正常组,差异具有统计学意义(P<0.05)。结论孕前超重和肥胖会不同程度的影响妊娠结局,因此在产前针对性的进行体育锻炼和营养均衡具有较好的临床效果,同时能够降低妊娠不良结局发生率,在临床应用中提供理论指导。
Objective To explore the effect of pre-pregnancy obesity on pregnancy outcome. Methods From January 2012 to December 2014, 2410 pregnant women undergoing prenatal examination and delivery in Shenzhen MCH were selected. Among them, 1777 were normal women, accounting for 73.73%; 498 were overweight women, accounting for 20.66% , 135 maternal obesity group, the rate was 5.60%. According to their body mass index, they were divided into normal group, overweight group and obesity group, and the corresponding statistical analysis was conducted to investigate the influence of prenatal obesity on pregnant women’s pregnancy outcome. Results The incidences of pregnancy-induced hypertension, diabetes mellitus, premature rupture of membranes, cesarean section rate and the incidence of postpartum hemorrhage in overweight and obesity group were significantly higher than those in normal group, and obese group was significantly higher than that in overweight group, the difference was statistically significant (P <0.05). The rate of fetal overweight in obese group was significantly higher than that in normal group and overweight group. The preterm birth rate in overweight group and obesity group was significantly higher than that in normal group. The incidence of respiratory distress in delivery group was significantly lower than that in normal group Statistical significance (P <0.05). Conclusion Overweight and obesity before pregnancy may affect pregnancy outcomes to varying degrees. Therefore, it is of great clinical effect in prenatal physical exercise and nutrition balance, and can reduce the incidence of adverse pregnancy outcomes and provide theoretical guidance in clinical application.
