目的:探讨三维虚拟手术规划技术对肝细胞癌精准肝切除的指导价值。方法:选择2016年1月-10月期间于我院治疗的14例肝细胞癌患者作为研究对象,所有患者均在应用三维虚拟手术规划技术后进行肝癌切除治疗。首先进行CT检查,之后对CT图像进行三维重建,对病灶进行定位、分割,评估患者临床情况,并以此结果制定个性化手术方案,将术前预计切除肝脏体积数值与术后实际切除肝脏体积进行对比。结果:三维虚拟手术规划技术重建的模型结构立体、清晰、逼真,14例患者术后残肝体积均得到准确评估,且术后仅出现1例切口感染并发症,未见肝功能衰竭、死亡病例。结论:三维虚拟手术规划技术可在术前准确模拟肝脏手术情况,对临床制定手术方案具有指导作用。
Objective: To explore the guiding value of three-dimensional virtual surgery planning technique for accurate hepatectomy of hepatocellular carcinoma. Methods: Fourteen patients with hepatocellular carcinoma treated in our hospital from January to October 2016 were enrolled in this study. All patients underwent resection of liver cancer after applying 3D virtual surgical planning technique. First of all, the CT examination was carried out. After that, the CT images were reconstructed in three dimensions, the lesions were located and divided, and the clinical situation of the patients was evaluated. Based on the results, a personalized surgical protocol was established. The value of the expected liver resection volume and the actual resection volume comparing. Results: The model structure of three-dimensional virtual surgical planning was reconstructed stereoscopically, clearly and vividly. All the residual volume of postoperative liver in 14 patients were accurately evaluated. Only 1 incision infection complication occurred after operation, and no liver failure and death were found . Conclusion: The three-dimensional virtual surgery planning technique can accurately simulate the operation of liver surgery before surgery, which can guide the clinical planning of surgical procedures.
