目的探讨CD55、CD59检测对阵发性睡眠性血红蛋白尿症(paroxysmal nocturnal hemoglobinuria,PNH)的临床诊断意义。方法采用流式细胞术检测20例PNH患者、45例AA患者和20例正常人外周血粒、红细胞CD55、CD59表达阳性率。结果正常人外周血粒、红细胞CD55、CD59表达阳性率均>95%;部分AA患者外周血粒、红细胞CD55、CD59表达阳性率<95%但均>90%;PNH患者外周血粒、红细胞CD55、CD59表达阳性率与正常对照组和AA组相比均明显减低,差异具有显著性(P<0.01)。结论外周血粒、红细胞CD55、CD59检测是诊断PNH较特异而又敏感的方法 ,特别是对表现不典型的PNH及AA-PNH综合征的诊断具有重要意义。
Objective To investigate the clinical significance of CD55 and CD59 in the diagnosis of paroxysmal nocturnal hemoglobinuria (PNH). Methods Flow cytometry was used to detect the positive rate of CD55 and CD59 expression in 20 PNH patients, 45 AA patients and 20 healthy controls. Results The positive rates of CD55 and CD59 expression in peripheral blood of normal people were> 95%. The positive rates of CD55 and CD59 expression in peripheral blood and erythrocytes in some AA patients were all <95%, but both were> 90%. The peripheral blood erythrocyte CD55 The positive rate of CD59 expression was significantly lower than that of normal control group and AA group (P <0.01). Conclusion The detection of CD55 and CD59 in peripheral blood and erythrocytes is a specific and sensitive method for the diagnosis of PNH, especially for the diagnosis of atypical PNH and AA-PNH syndrome.
