目的研究 2型糖尿病合并高血压患者血清瘦素水平的变化。 方法随机选取 2型糖尿病患者 5 5例 ,2型糖尿病合并高血压 30例 ;正常对照组 15例。分别检测以上三组患者的空腹瘦素、空腹和馒头餐后 2h血糖和胰岛素、糖化血红蛋白、体重指数 (BMI)、腰臀比值 (WHR)、血压、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白、载脂蛋白A、载脂蛋白B和脂蛋白 (a) ,并计算胰岛素敏感指数 (ISI)。 结果糖尿病合并高血压组的血清瘦素水平明显高于其它两组 ,血糖控制的差异对瘦素没有产生影响 ,高血压、胰岛素水平增高和敏感性下降、以及病程、BMI、WHR、性别和ApoB等因素均对瘦素有影响 ,正常对照组的胰岛素敏感性也随着年龄的上升而下降 ,但这与瘦素水平无关。结论在 2型糖尿病患者中血清瘦素受到高血压、高胰岛素血症、胰岛素抵抗、体脂分布、肥胖和血脂等多因素的影响 ,但与血糖水平无关。
Objective To study the changes of serum leptin levels in type 2 diabetic patients with hypertension. Methods Fifty-five patients with type 2 diabetes mellitus and 30 patients with type 2 diabetes mellitus and hypertension were randomly selected and 15 normal controls. The levels of fasting leptin, fasting and steamed bread 2h postprandial blood glucose and insulin, glycosylated hemoglobin, body mass index (BMI), WHR, blood pressure, total cholesterol, triglycerides, high density lipoprotein , Low density lipoprotein, apolipoprotein A, apolipoprotein B and lipoprotein (a), and calculate insulin sensitivity index (ISI). Results Serum leptin levels were significantly higher in diabetic patients with hypertension than those in the other two groups. The difference in blood glucose control had no effect on leptin, hypertension, increased insulin and decreased sensitivity, and duration, BMI, WHR, sex and ApoB And other factors have an impact on leptin, insulin sensitivity in the normal control group also decreased with age, but this has nothing to do with leptin levels. Conclusions Serum leptin is affected by multiple factors such as hypertension, hyperinsulinemia, insulin resistance, body fat distribution, obesity and blood lipids in patients with type 2 diabetes, but not with blood glucose levels.
