目的探讨γ-干扰素释放试验(TB-IGRA)在维吾尔族患者中结核病诊断和筛查中的诊断价值。方法分析2015年1月-2016年3月430例在我院接受治疗的疑似结核病患者的临床资料,其中维吾尔族患者226例,列为观察组,其余204例汉族患者设为对照组。结果维吾尔族患者TB-IGRA检测阳性率明显高于对照组,维吾尔族IFN-γ分泌水平均明显高于汉族(P<0.05);吾尔族女性TB-IGRA检测阳性率明显高于维吾尔族男性、汉族女性(P<0.05);维吾尔族20岁至30岁之间患者TB-IGRA检测阳性率明显高于汉族20岁至30岁之间患者TB-IGRA(P<0.05);TB-IGRA检测灵敏度、准确度明显高于痰涂片检测,差异均具有统计学意义(P<0.05)。结论 TB-IGRA检测在灵敏度和准确性明显优于涂片分枝杆菌检测,具有较高的结核病临床诊断价值。
Objective To investigate the diagnostic value of γ-interferon release test (TB-IGRA) in the diagnosis and screening of tuberculosis in Uygur patients. Methods Clinical data of 430 suspected tuberculosis patients treated in our hospital from January 2015 to March 2016 were analyzed. Among them, 226 cases of Uyghur were classified as observation group and the other 204 Han patients as control group. Results The positive rate of TB-IGRA in Uighur patients was significantly higher than that in the control group, and the level of IFN-γ secretion in Uighur was significantly higher than that in Han (P <0.05). The positive rate of TB-IGRA in Uighur women was significantly higher than that of Uighur men , And Han women (P <0.05). The positive rate of TB-IGRA in Uighur patients between 20 and 30 years old was significantly higher than that in Han nationality between 20 and 30 years old (P <0.05) Sensitivity and accuracy were significantly higher than sputum smear detection, the differences were statistically significant (P <0.05). Conclusion The sensitivity and accuracy of TB-IGRA test are significantly better than that of smear Mycobacterium smegmatis test, which has a high diagnostic value of TB.
