布地奈德混悬液雾化吸入辅助治疗婴幼儿毛细支气管炎的最佳剂量研究

目的:探讨不同剂量布地奈德(Budesonide,BUD)混悬液雾化吸入对于婴幼儿毛细支气管炎进行治疗的临床效果。方法:选取我院2013年6月-2014年6月间收治的120例毛细支气管炎患儿为研究对象,采用随机数字表法将患儿随机分为高剂量组(44例)、低剂量(40例)和对照组(36例)三组。对照组进行综合治疗,高剂量组和低剂量组在综合性治疗的基础上分别给予BUD混悬液1 mg/次与0.5 mg/次治疗。观察三组患儿不良反应的发生情况,并比较他们的气促缓解时间、咳嗽消失时间、哮鸣音消失时间以及住院时间是否存在差异。结果:高剂量组的气促缓解时间、咳嗽消失时间、哮鸣音消失时间以及住院时间均明显低于低剂量组和对照组,差异均有统计学意义(P<0.05);低剂量组的气促缓解时间、咳嗽消失时间、哮鸣音消失时间以及住院时间明显低于对照组,差异均有统计学意义(P<0.05)。高剂量组出现3例鹅口疮(6.82%),低剂量组出现2例(5.00%),两组比较,差异无统计学意义(x2=0.124,P=0.725),且经对症处理后很快治愈,三组未见其他不良反应。结论:高剂量BUD混悬液雾化吸入佐治患儿毛细支气管炎具有疗效好,成本低和安全性高的特点,但临床应注意患者不良反应的发生。 Objective: To investigate the clinical effect of inhalation of Budesonide (BUD) suspension at different doses on bronchiolitis in infants. Methods: A total of 120 children with bronchiolitis who were admitted to our hospital from June 2013 to June 2014 were selected as study subjects. The children were randomly divided into high dose group (44 cases), low dose group 40 cases) and control group (36 cases) three groups. The control group was treated by combination of high dose group and low dose group on the basis of comprehensive treatment, respectively BUD suspension 1 mg / time and 0.5 mg / time treatment. Adverse reactions were observed in three groups of children, and compared their duration of relief, cough disappear time, wheezing disappearance of time and hospital stay differences. Results: The relief time, cough disappearance time, disappearance time of wheeze and length of hospital stay in high-dose group were significantly lower than those in low-dose group and control group (P <0.05); low-dose group Shortness of breath, cough disappear time, wheeze disappearance time and hospitalization time were significantly lower than the control group, the difference was statistically significant (P <0.05). There were 3 thrush (6.82%) in the high dose group and 2 cases (5.00%) in the low dose group. There was no significant difference between the two groups (x2 = 0.124, P = 0.725) Cure, no other adverse reactions in the three groups. Conclusion: The treatment of bronchiolitis in children with high dose BUD suspension by nebulization has the advantages of good curative effect, low cost and high safety. However, patients should pay attention to the occurrence of adverse reactions in patients.
