目的探讨FUS-100全自动尿沉渣分析仪(简称FUS-100)中的细菌和酵母定量计数在筛查住院患者尿路感染时的价值。方法用定量细菌培养法和FUS-100检测505例疑似泌尿系统感染患者清洁中段尿标本。结果 505份尿标本培养阳性192份,阳性率为38.0%,其中36份标本有两种细菌生长,共分离出228株菌,革兰阳性球菌30株(13.2%),革兰阴性杆菌108株(47.4%),真菌90株(39.4%)。以尿细菌计数≥23.72/μL、酵母计数≥0.65/μL为感染阳性标准,与中段尿培养相比较,FUS-100细菌定量计数的敏感性为85.5%,特异性为89.5%,阳性似然比为8.14,阴性似然比为0.16;酵母定量计数的敏感性为80.4%,特异性为97.2%,阳性似然比为28.71,阴性似然比为0.20。结论 FUS-100定量计数尿细菌和酵母可用于住院患者尿路感染的快速筛查。
Objective To investigate the value of bacterial and yeast quantification in the FUS-100 Automatic Urinary Sediment Analyzer (FUS-100) for the screening of urinary tract infections in hospitalized patients. Methods Quantitative bacterial culture and FUS-100 were used to detect the clean middle urine specimens of 505 patients with suspected urinary tract infection. Results A total of 192 positive samples were obtained from a total of 192 urine specimens. The positive rate was 38.0%. There were two kinds of bacteria growth in 36 specimens, 228 strains were isolated, 30 (13.2%) were Gram-positive cocci, 108 were Gram- (47.4%) and 90 fungi (39.4%). The urinary bacterial count ≥23.72 / μL and the yeast count ≥0.65 / μL were positive for infection. Compared with the middle urine culture, the sensitivity and the specificity of FUS-100 were 85.5% and 89.5% respectively. The positive likelihood ratio Was 8.14, the negative likelihood ratio was 0.16; the sensitivity of yeast quantitative counting was 80.4%, the specificity was 97.2%, the positive likelihood ratio was 28.71 and the negative likelihood ratio was 0.20. Conclusion FUS-100 quantitative counting of urine bacteria and yeast can be used for rapid screening of urinary tract infections in hospitalized patients.
