目的了解社区围产保健孕妇的被动吸烟情况,并探讨其影响因素,为减少云南妇女烟草相关健康风险项目的下一步干预策略提供依据。方法 2014年4-5月对云南省内2个社区的432名孕妇进行面对面问卷调查,有效问卷426份,有效率为98.6%。采用SPSS19.0统计软件进行统计描述、χ2检验和非条件Logistic回归分析。结果孕妇无人吸烟,被动吸烟率为58.5%,孕妇被动吸烟的主要场所依次为自己家、亲戚朋友家和工作场所。多因素Logistic回归分析其影响因素,结果显示,二手烟意识和家中有吸烟者是影响孕妇被动吸烟的主要因素。知晓二手烟,家中有吸烟者,则孕妇被动吸烟率越高(OR=3.384,P=0.000;OR=3.293,P=0.000),但现居地、民族、年龄、孕周和文化程度等因素差异无统计学意义(P>0.05)。结论社区围产期保健孕妇的被动吸烟率较高。家中有吸烟者是被动吸烟干预中需要重点关注的人群。加强市/县/区一级妇幼保健机构的服务提供者的干预能力,通过有针对性的健康教育,让更多孕妇知晓二手烟及其二手烟暴露的危害,并促进家中的丈夫和吸烟者改变吸烟习惯,建立家中无烟策略。
Objective To understand the passive smoking status of pregnant women in perinatal health care community and to explore the influencing factors so as to provide the basis for further strategies to reduce tobacco-related health risks among women in Yunnan Province. Methods From April to May 2014, 432 pregnant women from 2 communities in Yunnan Province were interviewed by face-to-face questionnaire. There were 426 valid questionnaires with an effective rate of 98.6%. SPSS 19.0 statistical software for statistical description, χ2 test and non-conditional Logistic regression analysis. Results no smoking pregnant women, passive smoking was 58.5%, the main place of passive smoking of pregnant women, followed by their own home, relatives and friends at home and the workplace. Multivariate logistic regression analysis of its influencing factors, the results show that awareness of secondhand smoke and smokers at home is a major factor affecting passive smoking in pregnant women. If you know that you have secondhand smoke and have smokers in your home, the passive smoking rate of pregnant women is higher (OR = 3.384, P = 0.000; OR = 3.293, P = 0.000), but the factors such as current residence, ethnicity, age, gestational age and education level The difference was not statistically significant (P> 0.05). Conclusions The rate of passive smoking among pregnant women in perinatal community is relatively high. Home smokers are the people who need to focus their attention on passive smoking interventions. Strengthen the intervention capacity of service providers in MCH institutions at city, county and district levels to make more pregnant women aware of the dangers of secondhand smoke and secondhand smoke exposure through targeted health education and to promote husbands and smokers at home Change smoking habits and establish a smoke-free strategy in your home.
