外淋巴瘘的形成有向外破裂及向内破裂两种机制。向外破裂由于各种刺激引起中枢静脉压及脑脊液压力增加,通过外淋巴导管传向外淋巴所致。向内破裂系由于行Valsalva法时升高之空气压力,经咽鼓管达中耳所致。作者报告2例。例1:38岁女性患者,因耳硬化症行右镫骨底板切除术,术后感眩晕、恶心,术后第四日症状增剧,又两日后症状逐渐减轻,但患耳听力下降,术后16日出院时仍有耳鸣与平衡失调,且每当清理耳道时出现眩晕,擤鼻时亦可发生旋转感。三个月后复查听力骨导在40dB水平,气导近
