宫颈液基薄层细胞学检查10881例临床分析

目的:探讨液基薄层细胞学(LCT)在宫颈病变诊断中的意义,寻找提高子宫颈癌前病变及宫颈癌检出率的有效途径。方法:回顾性分析10 881例妇女宫颈液基细胞学检查结果,细胞学诊断采用TBS分级系统。结果:无上皮内病变或恶性病变(NILM)10 257例,占94.265%。LCT异常检出率为5.735%(624/10 881),其中非典型鳞状细胞(ASC-US)381例,占3.575%;低级别鳞状上皮内病变(LSIL)158例,占1.452%;高级别鳞状上皮内病变(HSIL)45例,占0.414%;非典型鳞状细胞,不除外高级别鳞状上皮内病变(ASC-H)8例,占0.074%;鳞状细胞癌(SCC)9例,占0.083%。非典型腺细胞(AGC)21例,占0.193%;腺癌(GCC)2例,占0.018%。结论:LCT技术和TBS诊断系统能提高子宫颈癌前病变和早期浸润癌的检出率。 Objective: To investigate the significance of liquid-based thin-layer cytology (LCT) in the diagnosis of cervical lesions and to find an effective way to improve the detection rate of cervical precancerous lesions and cervical cancer. Methods: A retrospective analysis of 10 881 women cervical liquid cytology test results, cytological diagnosis using TBS classification system. RESULTS: There were 10 257 cases with no intraepithelial lesion or malignant transformation (NILM), accounting for 94.265%. The detection rate of LCT was 5.735% (624/10881), of which 381 cases were ASC-US (accounting for 3.575%). There were 158 cases (1.452%) of LSIL with low-grade squamous intraepithelial lesion (LSIL) There were 45 cases of high-grade squamous intraepithelial lesion (HSIL), accounting for 0.414%. In atypical squamous cells, 8 cases (0.074%) were high-grade squamous intraepithelial lesion (ASC-H) ) 9 cases, accounting for 0.083%. There were 21 cases of atypical gland cells (AGC) accounting for 0.193%, 2 cases of adenocarcinoma (GCC) accounting for 0.018%. Conclusion: LCT and TBS diagnostic system can improve the detection rate of cervical precancerous lesions and early invasive carcinoma.
