目的建立实时三维超声心动图(RT-3DE)测定右心室容量及收缩功能的方法学,并评价其准确性。方法10头正常上海种白猪,出生2个月。除基础状态外,又通过中心静脉注水和动脉抽血方式依次制造动物的高血容量状态和低血容量状态,在各状态下应用RT-3DE胸骨旁大动脉短轴观为初始切面进行采图,以8平面法测定右室容量,计算右室每搏量(RVSV),测值与Swan-Ganz肺动脉热稀释法(PATD)比较。结果基础状态下,RT-3DE测定右室舒张末期容量(RVEDV)为(32.05±14.53)ml,右室收缩末期容量(RVESV)为(11.88±7.94)ml,计算RVSV为(20.13±7.48)ml,PADT同步测定的RVSV为(29.07±6.12)ml;高血容量状态下,RT-3DE测定RVEDV和RVESV的测值分别为(44.88±9.19)ml和(15.48±3.59)ml,计算RVSV为(29.40±8.37)ml,PADT同步测得的RVSV为(36.41±7.20)ml;低血容量状态下,RT-3DE测定RVEDV和RVESV的测值分别为(13.37±6.04)ml和(5.67±3.90)ml,计算RVSV为(7.70±3.22)ml,PADT同步测得的RVSV为(10.02±2.48)ml。RT-3DE在各状态下测定的RVSV与PADT同步测定的RVSV值之间均存在良好的相关性,相关系数分别为0.71、0.98和0.85,P值分别为0.048、0.001和0.015。结论在胸骨旁部位采用RT-3DE8平面法可以较准确地测定右室的容量及评价右室收缩功能。
Objective To establish a real-time three-dimensional echocardiography (RT-3DE) determination of right ventricular volume and systolic function of the methodology, and evaluate its accuracy. Methods 10 normal Shanghai white pigs, born 2 months. In addition to the basic state, and then by central venous infusion and arterial blood drawing in order to create the state of hypercapnia and hypovolemia status of animals, the application of RT-3DE in all states of the short axis view of the parasternal sternum as the initial section of mining, Right ventricular volume was measured by the 8-plane method and right ventricular stroke volume (RVSV) was calculated. The measured value was compared with Swan-Ganz pulmonary artery thermodilution method (PATD). Results RVEDV was (32.05 ± 14.53) ml, RVESV was (11.88 ± 7.94) ml, and RVSV was (20.13 ± 7.48) ml (29.07 ± 6.12) ml. The RVEDV and RVESV values measured by RT-3DE were (44.88 ± 9.19) ml and (15.48 ± 3.59) ml respectively in high blood volume state. The calculated RVSV was ( (13.37 ± 6.04) ml and (5.67 ± 3.90), respectively. The measured values of RVEDV and RVESV by RT-3DE were (29.37 ± 6.04) ml and ml, calculated RVSV was (7.70 ± 3.22) ml, PADT measured RVSV was (10.02 ± 2.48) ml. There was a good correlation between RVSV measured by RT-3DE and RVSV measured simultaneously by PADT. The correlation coefficients were 0.71, 0.98 and 0.85 respectively, and P values were 0.048, 0.001 and 0.015 respectively. Conclusion RT-3DE8 planar method can be used to determine the volume of right ventricle and evaluate the right ventricular systolic function at the sternum.
