目的探讨彩色多普勒超声在经腋静脉入路置入植入式输液港的临床应用价值。方法选择需长期化疗的各类肿瘤患者122例,在超声引导下行经腋静脉入路置入植入式输液港。计算穿刺成功率,比较一次穿刺成功与二、三次穿刺成功腋静脉的宽度,记录手术操作时间、植入式输液港植入后并发症的发生率。结果 122例均顺利经腋静脉穿刺置入植入式输液港,其中一次穿刺成功102例,二次穿刺成功15例,三次穿刺成功5例。一次穿刺成功腋静脉宽度与二、三次穿刺成功腋静脉的宽度比较有统计学意义(P<0.05)。122例置管平均时间为(18±3.6)min。1例血液病患者出现穿刺部位血肿,2例出现输液不畅,1例术后出现感染。结论超声在经腋静脉入路置入植入式输液港有重要的应用价值。
Objective To investigate the clinical value of color Doppler ultrasound in implantable transfusion via axillary vein. Methods A total of 122 patients with various types of cancer requiring long-term chemotherapy were enrolled in the study. The success rate of puncture was calculated. The success of puncture and the width of axillary vein after two and three punctures were compared. The operation time and the incidence of postoperative complications were recorded. Results All of the 122 cases were successfully transplanted into the implantation port through the axillary vein puncture. Among them, 102 cases were successful in one puncture, 15 cases succeeded in secondary puncture, and 5 cases succeeded in three punctures. A successful puncture axillary vein width and two or three punctures the width of axillary vein was statistically significant (P <0.05). The mean duration of catheterization in 122 cases was (18 ± 3.6) min. One patient with hematologic disease showed puncture site hematoma, 2 patients had poor infusion and 1 patient had postoperative infection. Conclusion Ultrasound through the axillary vein into the implantable transfusion port has an important value.
