目的:研究通过小骨窗微创脑出血清除术治疗高血压脑出血的临床效果。方法:选择广州市番禺中心医院自2012年1月至2016年1月治疗的高血压脑出血患者100例,依照随机对照原则分为观察组和对照组,各50例。对照组患者治疗方案选用大骨瓣开颅血肿清除术,观察组患者采用小骨窗微创脑出血清除术。从临床效果、死亡率、并发症发生率以及手术前后神经功能缺损情况、术后6个月随访等,对两组患者进行对比分析。结果:在治疗有效率对比中,观察组明显高于对照组,差异具有统计学意义(P<0.05);两组患者死亡率对比中,观察组明显低于对照组,差异具有统计学意义(P<0.05);相关并发症发生率方面对比中,观察组明显低于对照组,差异具有统计学意义(P<0.05);术后NIHSS评分对比中,观察组评分低于对照组,差异具有统计学意义(P<0.05),在日常生活能力对比中,观察组患者明显优于对照组患者,差异具有统计学意义(P<0.05)。结论:对高血压脑出血的患者选择小骨窗微创脑出血清除术进行手术能有效提高疗效,改善患者预后。
Objective: To study the clinical effect of minimally invasive intracerebral hemorrhage surgery on hypertensive intracerebral hemorrhage through small bone window. Methods: One hundred patients with hypertensive intracerebral hemorrhage who were treated in Panyu Central Hospital of Guangzhou City from January 2012 to January 2016 were randomly divided into observation group and control group, with 50 cases in each group. Patients in the control group were treated with large craniotomy and hematoma resection. Patients in the observation group were treated with minimally invasive brain debridement. From the clinical effects, mortality, complications and neurological deficits before and after surgery, follow-up after 6 months, etc., the two groups were compared. Results: Compared with the control group, the observation group was significantly higher than that of the control group (P <0.05). In the comparison of the mortality rate of the two groups, the observation group was significantly lower than the control group, the difference was statistically significant (P < P <0.05). Comparing the incidence of complications, the observation group was significantly lower than the control group, the difference was statistically significant (P <0.05); NIHSS score postoperative comparison, the observation group score was lower than the control group, the difference was Statistical significance (P <0.05), in the daily living ability comparison, the observation group was significantly better than the control group patients, the difference was statistically significant (P <0.05). Conclusion: It is effective for patients with hypertensive intracerebral hemorrhage to choose minimally invasive brain debridement surgery for small bone window to improve the curative effect and prognosis.
