目的观察并比较羟基喜树碱联合mFOLFOX6方案与单纯mFOLFOX6方案治疗晚期胃癌的临床疗效和毒副反应。方法 66例晚期胃癌患者随机分为观察组和对照组,对照组给予mFOLFOX6方案奥沙利铂+四氢叶酸+5-氟尿嘧啶,观察组额外给予羟基喜树碱,均以2周为1周期,至少接受2个周期化疗。评价两组的近期疗效、远期疗效和毒副反应。结果观察组和对照组有效率分别为52.94%和40.63%,差异有统计学意义(P<0.05)。两组1年生存率分别为91.18%和90.63%,差异无统计学意义(P>0.05),两组3年生存率分别为67.75%和56.25%,差异有统计学意义(P<0.05)。观察组腹泻发生率为38.24%,高于对照组的发生率(18.75%),差异有统计学意义(P<0.05),其他毒副反应两组比较差异均无统计学意义(P>0.05)。结论与单纯mFOLFOX6方案比较,羟基喜树碱联合mFOLFOX6方案治疗晚期胃癌能明显提高近、远期疗效,并未明显增加毒副反应,但在用药过程中应注意腹泻的发生。 Objective To observe and compare the clinical efficacy and side effects of hydroxycamptothecin combined with mFOLFOX6 regimen and simple mFOLFOX6 regimen in the treatment of advanced gastric cancer. Methods Sixty-six patients with advanced gastric cancer were randomly divided into observation group and control group. The control group was given oxaliplatin + tetrahydrofolate + 5-fluorouracil with mFOLFOX6 regimen. The observation group was additionally given hydroxycamptothecine, At least 2 cycles of chemotherapy. Evaluation of the two groups of short-term efficacy, long-term efficacy and toxicity. Results The effective rates of the observation group and the control group were 52.94% and 40.63%, respectively, with significant differences (P <0.05). The 1-year survival rates of the two groups were 91.18% and 90.63% respectively, with no significant difference (P> 0.05). The 3-year survival rates of the two groups were 67.75% and 56.25%, respectively, with statistical significance (P <0.05). The incidence of diarrhea in the observation group was 38.24%, higher than that in the control group (18.75%), the difference was statistically significant (P <0.05). There was no significant difference in the other toxic and side effects between the two groups (P> 0.05) . Conclusion Compared with the simple mFOLFOX6 regimen, hydroxycamptothecin combined with mFOLFOX6 regimen can significantly improve the short-term and long-term curative effects without increasing the side effects. However, diarrhea should be noticed during the course of medication.
