目的系统评价血管内皮生长因子(VEGF)在中国三阴性乳腺癌(TNBC)患者中的表达情况以及其阳性表达与TNBC患者临床病理特征的关系。方法通过中国知网、万方数据库、维普、中国生物医学数据库、超星Medalink和Pub Med数据库检索已发表的有关关于中国TNBC患者的相关文献资料,由2名研究者按照纳入与排除标准独立筛选文献、提取资料和文献评价,然后使用Rev Man 5.3软件进行meta分析。结果本研究纳入11篇相关文献共1 838例患者,其中TNBC患者750例,非TNBC患者1 088例。meta分析结果显示,VEGF蛋白表达阳性率在TNBC患者中明显高于非TNBC患者[OR=2.64,95%CI(2.14,3.26),P<0.000 01]。在TNBC患者中,VEGF蛋白表达阳性率在淋巴结(+)患者中明显高于淋巴结(–)患者[OR=0.30,95%CI(0.14,0.46),P=0.000 2]、在Ⅲ–Ⅳ期患者中明显高于Ⅰ–Ⅱ期患者[OR=0.43,95%CI(0.29,0.62),P<0.000 01],其与TNBC患者的年龄[OR=0.80,95%CI(0.54,1.19),P=0.27]和肿瘤大小[OR=0.84,95%CI(0.53,1.33),P=0.46]无关。结论 VEGF蛋白在TNBC患者中呈高表达,有望成为其治疗的新靶点;且其蛋白阳性表达与TNBC患者的阳性淋巴结及TNM分期晚有关,其可能与TNBC患者的预后有一定的关系。
Objective To evaluate the expression of vascular endothelial growth factor (VEGF) in triple-negative breast cancer (TNBC) patients in China and its relationship with the clinicopathological features of TNBC. Methods The published literature about Chinese TNBC patients was searched by CNKI, Wanfang Database, VIP, China Biomedicine Database, Superstar Medalink and Pub Med database. Two researchers independently screened the literature according to inclusion and exclusion criteria , Extract data and literature reviews, and then use the Rev Man 5.3 software for meta-analysis. Results A total of 1 838 patients were enrolled in 11 related articles, including 750 in TNBC and 1 088 in non-TNBC. The meta-analysis showed that the positive rate of VEGF protein expression in TNBC patients was significantly higher than that in non-TNBC patients [OR = 2.64, 95% CI (2.14, 3.26), P <0.000 01]. In TNBC patients, the positive rate of VEGF protein expression was significantly higher in patients with lymph node (+) than in patients with lymph node (OR) (OR = 0.30, 95% CI 0.14, 0.46, P = 0.0002) Patients were significantly higher than those in patients with stage I-II [OR = 0.43, 95% CI (0.29, 0.62), P <0.000 01] P = 0.27] and tumor size [OR = 0.84, 95% CI (0.53, 1.33), P = 0.46]. CONCLUSION: VEGF protein is highly expressed in TNBC patients and may be a new target for its treatment. The positive expression of VEGF protein is related to the late stage of lymph node and TNM in patients with TNBC, which may be related to the prognosis of patients with TNBC.
