目的分析原发性脑内恶性淋巴瘤的CT、MR影像表现,提高对本病的诊断及鉴别诊断准确率。方法回顾分析8例经病理证实的原发性脑内恶性淋巴瘤的CT、MRI影像资料,其中6例进行CT检查,2例进行MRI检查。结果8例中有7例为单发,1例为多发,共9个病灶,其中位于脑表面及灰白质交界区病灶6个,位于脑室内深部白质区病灶3个。原发性脑内恶性淋巴瘤的影像学病灶形态多为圆形或类圆形,边界较清楚,瘤周水肿及占位效应轻。CT表现为等或稍高密度影,无出血及钙化;MRI表现为T1WI呈略低或等信号,T2WI呈等或略高信号,注射对比剂后病灶均有强化。病理结果均为非霍奇金淋巴瘤。结论原发性脑内恶性淋巴瘤影像学表现及发生部位缺乏特征性,容易产生误诊,综合分析影像表现有助于提高对该病的诊断。
Objective To analyze the CT and MR imaging manifestations of primary intracerebral malignant lymphoma and to improve the accuracy of the diagnosis and differential diagnosis of this disease. Methods The CT and MRI images of 8 cases of pathologically confirmed primary intracerebral malignant lymphoma were retrospectively analyzed. Among them, CT was performed in 6 cases and MRI in 2 cases. Results Of the 8 cases, 7 were single and 1 were multiple. There were 9 lesions, of which 6 were located in the border of brain and gray matter, and 3 were located in deep white matter of brain. Primary intracerebral malignant lymphoma imaging lesions mostly round or round, border clear, peritubular edema and mass effect of light. CT showed equal or slightly higher density of shadow, no bleeding and calcification; MRI showed T1WI was slightly lower or equal signal, T2WI was equal or slightly higher signal, injection of contrast agent lesions were enhanced. Pathological findings were non-Hodgkin’s lymphoma. Conclusions The imaging manifestations of primary intracerebral malignant lymphoma and the lack of characteristic parts of them are prone to misdiagnosis. A comprehensive analysis of the imaging findings may help to improve the diagnosis of the disease.
