目的探讨给予多囊卵巢综合征(PCOS)患者二甲双胍联合克罗米酚治疗对血清性激素结合球蛋白(SHBG)及硫酸脱氢表雄酮(DHEAS)水平的影响。方法 50例PCOS患者,随机分为对照组及观察组,各25例。对照组给予克罗米酚治疗,观察组给予二甲双胍联合克罗米酚治疗。对比分析两组SHBG及DHEAS水平。结果治疗前,对照组SHBG为(57.75±1.30)nmol/L、DHEAS为(351.82±2.03)μg/ml,观察组SHBG为(57.77±1.47)nmol/L、DHEAS为(350.82±3.10)μg/ml;治疗后,对照组SHBG为(74.30±2.30)nmol/L、DHEAS为(190.80±3.04)μg/ml,观察组SHBG为(80.10±1.40)nmol/L、DHEAS为(180.80±2.35)μg/ml。治疗前两组SHBG、DHEAS水平对比,差异无统计学意义(t=0.051、1.349,P>0.05),治疗后两组SHBG水平均较本组治疗前升高,DHEAS水平均较本组治疗前降低,差异均具有统计学意义(tSHBG=31.321、55.000,tDHEAS=220.245、218.532,P<0.05),且治疗后观察组SHBG水平高于对照组,DHEAS水平低于对照组,差异均具有统计学意义(t=10.770、13.013,P<0.05)。结论给予PCOS患者二甲双胍联合克罗米酚治疗的效果显著,可改善性激素紊乱,降低DHEAS水平,升高SHBG水平,促进排卵,提高受孕率。
Objective To investigate the effects of metformin and clomiphene on the levels of serum sex hormone binding globulin (SHBG) and DHEAS in patients with polycystic ovary syndrome (PCOS). Methods 50 cases of PCOS patients were randomly divided into control group and observation group, 25 cases each. The control group was treated with clomiphene and the observation group was treated with metformin and clomiphene. The levels of SHBG and DHEAS were compared between the two groups. Results Before treatment, SHBG in the control group was (57.75 ± 1.30) nmol / L, DHEAS was (351.82 ± 2.03) μg / ml, SHBG in the observation group was (57.77 ± 1.47) nmol / L and DHEAS was (350.82 ± 3.10) ml.The SHBG in the control group was (74.30 ± 2.30) nmol / L, the DHEAS was (190.80 ± 3.04) μg / ml in the control group, the SHBG in the observation group was (80.10 ± 1.40) nmol / L and the DHEAS was (180.80 ± 2.35) μg / ml. The levels of SHBG and DHEAS in the two groups before treatment were not significantly different (t = 0.051, 1.349, P> 0.05). After treatment, the SHBG levels in both groups were significantly higher than those before treatment, and the DHEAS levels were significantly higher than those before treatment (TSHBG = 31.321, 55.000, tDHEAS = 220.245, 218.532, P <0.05). After treatment, the SHBG level in the observation group was higher than that in the control group, and the level of DHEAS was lower than that in the control group Significance (t = 10.770, 13.013, P <0.05). Conclusions Metformin combined with clomiphene is effective in PCOS patients. It can improve the sex hormones, reduce the level of DHEAS, increase the level of SHBG, promote the ovulation and increase the pregnancy rate.
