目的探讨2型糖尿病患者瘦素水平与认知功能的关系。方法对85例2型糖尿病患者进行认知问卷,评定认知功能,并测定患者血清瘦素、胰岛素、血糖、血脂、促甲状腺激素(TSH)等水平。结果不同瘦素水平的2型糖尿病患者认知功能有统计学差异(P<0.05);瘦素与MMSE总分、MMSE即刻记忆1、MMSE执行能力、言语流畅性等呈明显负相关(P<0.05),与指图总耗时及图片误认数呈正相关(P<0.05);瘦素与性别、年龄、胰岛素抵抗指数(HOMA-IR)、TSH、体重指数(BMI)以及是否合并高血压、冠心病等也密切相关(P<0.05)。多元逐步回归分析显示,HOMA-IR、性别及BMI为影响瘦素水平最显著的因素。结论瘦素可加重2型糖尿病患者的认知功能损害;瘦素水平与HOMA-IR、性别和BMI等密切相关。
Objective To investigate the relationship between leptin level and cognitive function in type 2 diabetic patients. Methods Eighty - five patients with type 2 diabetes mellitus were enrolled in a cognitive questionnaire to assess cognitive function. Serum levels of leptin, insulin, blood glucose, serum lipids and thyrotropin (TSH) were measured. Results There was a significant difference in cognitive function between type 2 diabetic patients with different leptin levels (P <0.05). Leptin showed a significant negative correlation with total score of MMSE, MMSE immediate memory 1, MMSE performance and verbal fluency (P < (P <0.05). There was a positive correlation between leptin and gender, age, insulin resistance index (HOMA-IR), TSH, body mass index (BMI) , Coronary heart disease are also closely related (P <0.05). Multivariate stepwise regression analysis showed that HOMA-IR, sex and BMI were the most significant factors affecting leptin level. Conclusion Leptin can aggravate cognitive impairment in type 2 diabetic patients. Leptin level is closely related to HOMA-IR, sex and BMI.
