目的:观察清肺化痰方治疗小儿哮喘的临床疗效。方法:将符合研究标准的支气管哮喘发作期患儿随机分为治疗组与对照组,并设正常儿童组。治疗组服用清肺化痰方,对照组服用盐酸丙卡特罗口服液,两组疗程均为10 d。观察治疗前后临床症状及体征变化,评价中医证候积分与临床疗效;脉冲振荡肺功能检测呼吸道总阻力(R_5)、中心气道阻力(R_(20))、外周弹性阻力(X_5)及共振频率(Fres)。结果:治疗后两组患儿主要症状、体征积分均较治疗前有显著降低(P<0.01),其中治疗组在喘息、咯痰、哮鸣音方面的积分较对照组有明显下降(P<0.01),且治疗组的临床控制率显著高于对照组(P<0.05)。两组患儿治疗前的R_5、R_5-R_(20)均明显高于正常组(P<0.05),X5明显低于正常组(P<0.05);治疗后,两组患儿的肺功能指标均较治疗前有明显好转(P<0.05),其中治疗组对R_5、X_5的改善优于对照组(P<0.05),但两组的各项指标仍与正常组有显著性差异(P<0.05)。结论:清肺化痰方能改善哮喘患儿的临床表现,改善气道炎症与肺的顺应性。
Objective: To observe the clinical curative effect of Qingfei Huatan Fang on pediatric asthma. Methods: The children with bronchial asthma exacerbation meeting the research criteria were randomly divided into treatment group and control group, with normal children group. The treatment group took Qingfei Huatan side, the control group took Procaterol Hydrochloride oral liquid, the two courses of treatment were 10 d. The changes of clinical symptoms and signs before and after treatment were evaluated to evaluate the TCM syndrome score and clinical efficacy. The total respiratory resistance (R_5), central airway resistance (R_ (20)), peripheral elastic resistance (X_5) and resonance frequency (Fres). Results: After treatment, the main symptoms and signs of children in both groups were significantly lower than those before treatment (P <0.01), and the scores of asthma, expectoration and wheeze of the treatment group were significantly lower than those of the control group (P < 0.01), and the clinical control rate in the treatment group was significantly higher than that in the control group (P <0.05). Before treatment, the R_5 and R_5-R_ (20) in both groups were significantly higher than those in the normal group (P <0.05), while the X5 in the two groups were significantly lower than those in the normal group (P <0.05) (P <0.05). The improvement of R_5 and X_5 in the treatment group was better than that in the control group (P <0.05), but the indexes in both groups were still significantly different from those in the normal group (P < 0.05). Conclusion: Qingfei Huatan can improve the clinical manifestations of children with asthma and improve airway inflammation and pulmonary compliance.
