目的 观察雄黄、益髓解毒方病证结合治疗低、中危-1骨髓增生异常综合征临床疗效及安全性.方法 将23例MDS患者,根据危度分层辨病予不同剂量水飞雄黄(低危组:10 mg/kg/d;中危-1组:15 mg/kg/d)分装胶囊2粒.1粒1次,日2次口服,疗程6个月.同时辨证服用益髓解毒方.检测治疗前后外周血白细胞(WBC)、血红蛋白(Hb)、血小板(PLT)计数、T细胞亚群(CD4+、CD8+)数值,并评价其疗效;比较RCUD、RCMD、RAEB-1不同分型疗效、按国际预后积分系统(IPSS)低危、中危-1组疗效;治疗前后疾病危度分层划分例数.结果 23例患者治疗前后WBC、PLT比较,差异无统计学意义(P>0.05),Hb治疗前后比较差异有统计学意义(P0.05);治疗前后危度分层划分例数比较差异有统计学意义(P<0.05).安全性观察:血砷浓度波动于10.3~21.6 ng/mL,无不良反应.结论 雄黄、益髓解毒方病证结合治疗MDS,对骨髓红系作用确切,可减低危度等级,延缓疾病进展,调节细胞免疫,安全有效.
