尿道成形术方法颇多,我们采用带蒂阴囊正中皮瓣治疗尿道下裂及前尿道广泛性狭窄10例,全部随访,获满意效果。现报告如下: 手术方法一、尿道下裂修复术 1.阴茎伸直,矫正下曲。于阴茎腹侧正中两旁0.5cm处纵行平行切开皮肤,深达阴茎白膜,上至冠状沟,下达尿道下裂口,提起一侧皮瓣游离以彻底切除尿道纤维索组织为止,使阴茎伸直。 2.扩大尿道下裂口,纵行切开呈“V”,刚好与纵行平行切口线相接,并插入合适的尿管。 3.阴囊皮肤尿道成形术,将阴茎向阴囊牵拉翻转,与阴囊正中皮瓣切线重叠,用5—0肠线将阴茎切线皮瓣内缘与阴囊切线皮瓣内缘连续缝合,固定于阴囊筋膜表面。然后再用1号丝线间断缝合阴茎与阴囊皮肤外缘、消灭创缘。 4.待7天后,拆除皮肤丝线。距缝合处
