目的 通过对重度阻塞性睡眠呼吸暂停 (OSA)患者夜间心率变异性的分析 ,以了解OSA患者夜间睡眠时植物神经功能状态。方法 对 2 1例重度阻塞性睡眠呼吸暂停患者及 2 0例正常受试者分别进行夜间 7小时多导睡眠呼吸监测及心率变异性分析。结果 OSA患者与对照组相比AHI,最低血氧 ,平均血氧 ,氧饱和度低于 90 %百分比 ,心率变异性指标SDNN ,CV ,LF/HF差异均具有显著性。且氧饱和度低于 90 %百分比与LF/HF呈正相关。结论 重度OSAS患者夜间存在严重低氧以及心率变异性增大 ,提示重度OSAS患者迷走、交感神经张力均增高且以交感神经张力增高为主。低氧的持续时间与交感神经张力呈正相关。
Objective To analyze the nighttime heart rate variability in patients with severe obstructive sleep apnea (OSA) to understand the functional status of autonomic nerves in patients with OSA during nighttime sleep. Methods Twenty-one patients with severe obstructive sleep apnea and 20 normal subjects were monitored for polysomnography at 7 hours and heart rate variability at night. Results Compared with the control group, the AHI, the minimum oxygen saturation, the average oxygen saturation and the oxygen saturation of OSA patients were less than 90%, and the differences of heart rate variability index SDNN, CV, LF / HF were significant. And oxygen saturation below 90% percentage positively correlated with LF / HF. Conclusions Severe hypoxemia and increased heart rate variability occur in patients with severe OSAS at night, suggesting that both vagal and sympathetic tone are increased and sympathetic tone is predominant in patients with severe OSAS. The duration of hypoxia was positively correlated with sympathetic tone.
