[目的]观察桃红四物汤与针刺联合西药治疗糖尿病周围神经病变疗效。[方法]使用随机平行对照方法,将70例住院患者按抽签法简单随机分为两组。对照组35例常规治疗,降血糖药或胰岛素、合理饮食等。治疗组35例桃红四物汤(当归20g,熟地、川芎各15g,赤芍、桃仁、红花各10g),1剂/d,水煎200m L,早晚空腹口服;针刺:取三阴交、肾俞、足三里、膈俞穴斜刺1寸,使针感在局部或深部传导扩散;取四肢穴直刺1~1.5寸,使针感向四肢末端传导;以上穴位针刺强度要基于患者的忍受限度,针刺后留针30min;西药治疗同对照组。连续治疗15d为1疗程。观测临床症状、血糖、餐后2h血糖、血黏度、糖化血红蛋白、运动神经传导速度、感觉神经传导速度、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组显效14例,有效16例,无效5例,总有效率85.71%。对照组显效9例,有效10例,无效16例,总有效率54.29%。治疗组疗效优于对照组(P<0.05)。[结论]桃红四物汤、针刺联合西药治疗糖尿病周围神经病变效果显著,值得推广。
[Objective] To observe the therapeutic effect of Taohong Siwu Decoction and acupuncture combined with western medicine on diabetic peripheral neuropathy. [Methods] Using randomized parallel control method, 70 inpatients were randomly divided into two groups according to random sampling method. Control group of 35 cases of conventional treatment, hypoglycemic agents or insulin, a reasonable diet. The treatment group 35 cases of Taohongsiwutang (Angelica 20g, Rehmannia, Chuanxiong the 15g, red peony root, peach kernel, safflower each 10g), 1 / d, decoction 200m L, morning and evening fasting oral; acupuncture: Take Sanyinjiao, kidney Yu, Zusanli, Geshu hole oblique 1-inch, so that acupuncture points in the local or deep conduction diffusion; take four limbs straight piercing 1 to 1.5 inches, so that the sense of acupuncture to the extremities conduction; acupuncture acupuncture strength above points should be based on the patient’s Tolerance limit, needle 30min after acupuncture; Western medicine treatment with the control group. Continuous treatment 15d for a course of treatment. Observation of clinical symptoms, blood glucose, 2h postprandial blood glucose, blood viscosity, glycosylated hemoglobin, motor nerve conduction velocity, sensory nerve conduction velocity, adverse reactions. Continuous treatment of 2 courses to determine the efficacy. [Results] In the treatment group, 14 cases were markedly effective, 16 cases were effective and 5 cases were ineffective. The total effective rate was 85.71%. In the control group, 9 cases were markedly effective, 10 cases were effective and 16 cases were ineffective. The total effective rate was 54.29%. The treatment group was better than the control group (P <0.05). [Conclusion] Taohong Siwu Decoction and acupuncture combined with western medicine have significant effect in treating diabetic peripheral neuropathy and worthy of promotion.
