目的评价平板运动试验在儿童心律失常诊断中的应用价值及安全性。方法采用快速级数递增法(RIS法),对237例心律失常患儿进行平板运动试验,对其结果进行分析。结果158例室性早搏中,运动后早搏增加44例;17例I度房室传导阻滞中11例,Ⅱ度房室传导阻滞10例运动后均无变化;8例Ⅲ度房室传导阻滞运动后均未能达到预定心率,其中5例出现频发室性早搏或部分二联律;10例有室上性心动过速发作史,运动均未诱发;34例窦性心动过缓,运动后7例未达到预定心率。受试患儿中无昏厥和严重心律失常等并发症发生。结论平板运动试验是一项安全、有效的小儿心律失常诊断方法;RIS法可适用于3岁以上小儿。
Objective To evaluate the value and safety of treadmill exercise test in diagnosis of arrhythmia in children. Methods 237 patients with arrhythmia were subjected to treadmill exercise test by rapid progression method (RIS method), and the results were analyzed. Results Of the 158 cases of premature ventricular contractions, premature beats increased by 44 cases. No change occurred in 17 cases of I degree atrioventricular block and 10 cases of Ⅱ degree atrioventricular block. There were 8 cases of Ⅲ degree atrioventricular conduction After blocking the movement were unable to reach the predetermined heart rate, of which 5 cases of frequent premature ventricular contractions or partial bigeminy; 10 cases of supraventricular tachycardia attack history, exercise were not induced; 34 cases of sinus bradycardia , 7 cases did not reach the scheduled heart rate after exercise. No complications in children with syncope and serious arrhythmia occurred. Conclusion The treadmill exercise test is a safe and effective method for the diagnosis of pediatric arrhythmia. The RIS method is suitable for children over 3 years of age.
