目的探讨旋转铰链型人工膝关节在膝关节周围骨肉瘤保肢术中的临床应用效果。方法膝关节周围骨肉瘤患者21例,其中股骨远端15例,胫骨近端6例。对膝部骨肉瘤进行广泛切除或根治性切除后,采用旋转铰链型人工膝关节置换重建,同时采用新辅助化疗治疗。结果 21例患者平均随访12~120个月。其中16例患者无局部复发或远处转移迹象,5例出现复发;MSTS功能评分提示70%的患者各项评估在3分以上。结论对膝关节周围骨肉瘤采用旋转铰链型膝关节置换治疗是有效的保肢方法 ,能降低局部并发症及提高临床疗效。
Objective To investigate the clinical effect of revolute hinge artificial knee joint in salvage operation of osteosarcoma around the knee joint. Methods Twenty-one patients with osteosarcoma around the knee, including 15 cases of distal femur and 6 cases of proximal tibia. After extensive resection or radical resection of osteosarcoma of the knee, rotary knee arthroplasty was used to replace and reconstruct the knee osteosarcoma while neoadjuvant chemotherapy was used. Results 21 patients were followed up for an average of 12 to 120 months. Sixteen patients showed no signs of local recurrence or distant metastasis, and 5 patients developed recurrence. MSTS score indicated that 70% of the patients had a score of 3 or above. Conclusion Rotational knee arthroplasty is an effective method of limb salvage for osteosarcoma around knee joint. It can reduce the local complications and improve the clinical curative effect.
