目的 探讨白细胞介素 18(IL 18)在乙型肝炎病毒 (HBV)感染中的作用。方法 从 5 7例HBV感染者 (无症状携带者 15例 ,慢性肝炎患者 30例 ,重型肝炎患者 12例 )和 10例健康献血员血液中分离出外周血单个核细胞 (PBMC) ,在脂多糖 (LPS)刺激 6h后 ,应用逆转录聚合酶链反应 (RT PCR)及流式细胞术技术对IL 18基因的转录及表达水平进行检测。结果 不同临床类型HBV感染者之间IL 18基因转录及表达水平的差异均有非常显著意义 (P <0 .0 1) ,重型肝炎组IL 18基因转录及表达水平高于正常对照 (均P <0 .0 1) ,无症状携带者组低于正常对照组 (P <0 .0 5 ,P <0 .0 1) ,慢性肝炎组与正常对照组比较 ,差异无显著意义 (均P >0 .0 5 )。随着病情加重 ,IL 18水平上升。肝组织炎症活动度不同的慢性肝炎患者之间IL 18基因转录及表达水平的差异均有显著意义 (P <0 .0 5 ) ,肝组织炎症程度越重 ,IL 18水平越高。慢性肝炎组患者IL 18基因的转录及表达水平均与血清丙氨酸转氨酶含量呈明显正相关 (r=0 .5 4,P <0 .0 1;r=0 .6 3 ,P <0 .0 1)。无症状携带者、慢性肝炎患者、重型肝炎患者PBMC内IL 18mRNA表达水平与PBMC内IL 18蛋白含量呈高度直线正相关 (r=0 .980 ,P <0 .0 0 1;r=0 910 ,P <0 .0 0 1;r=0 .975 ,P <0 .0 0 1)。结论?
Objective To investigate the role of interleukin 18 (IL 18) in hepatitis B virus (HBV) infection. Methods Peripheral blood mononuclear cells (PBMCs) were isolated from blood of 57 HBV-infected individuals (15 asymptomatic carriers, 30 chronic hepatitis patients, 12 severe hepatitis patients) and 10 healthy blood donors. (LPS) for 6 h, the transcription and expression levels of IL 18 gene were detected by reverse transcription polymerase chain reaction (RT PCR) and flow cytometry (FCM). Results There were significant differences in the transcription and expression of IL 18 gene between HBV infected patients with different clinical types (P <0.01). The levels of IL 18 gene transcription and expression in severe hepatitis group were significantly higher than those in normal controls (P < 0. 0 1), asymptomatic carrier group was lower than the normal control group (P <0.05, P <0.01), there was no significant difference between the chronic hepatitis group and the normal control group (all P> 0 .0 5). As the condition progresses, IL-18 levels rise. There were significant differences in the transcription and expression of IL-18 between chronic hepatitis patients with different levels of liver inflammation activity (P <0.05), the more severe liver inflammation and the higher IL-18 levels. The transcription and expression level of IL 18 gene in patients with chronic hepatitis were positively correlated with serum alanine aminotransferase levels (r = 0.54, P <0.01; r = 0.63, P <0. 0 1). The expression of IL 18mRNA in peripheral blood mononuclear cells of asymptomatic carriers, chronic hepatitis patients and patients with severe hepatitis was highly linearly positively correlated with the level of IL 18 protein in PBMCs (r = 0.980, P <0.01; r = 0 910, P <0. 01; r = 0 .975, P <0. 001). in conclusion?
