目的:研究拔除上下颌第一前磨牙或上颌第一前磨牙、下颌第二前磨牙矫治对青少年安氏II类Ⅰ分类错软硬组织的影响,为临床治疗提供参考。方法:选择拔除上下颌第一前磨牙(A组)或上颌第一前磨牙、下颌第二前磨牙(B组)矫治的青少年病例各10例。两组样本的性别、年龄、疗程相互匹配。对两组治疗前后的头颅侧位片进行测量分析,研究不同拔牙模式治疗前后软硬组织的变化。所有数据均采用SPSS11.5软件包进行统计学处理。结果:两组病例矫治后U1-L1,S-GO,N-ANS,ANS-Me增加,U1-SN减小;Holdaway软组织分析显示:两组病例矫治后上唇沟深,鼻下点至H线距,H角,下唇至H线距,颏唇沟深度减小,两组差别无统计学意义。结论:两种拔牙方式均能有效内收上切牙,改善软组织侧貌。下颌拔牙位置对软组织侧貌的改变无明显影响。
Objective: To study the effect of removal of maxillary first premolar or maxillary first premolar and mandibular second premolar on soft and hard Class II malocclusions in adolescents, so as to provide reference for clinical treatment. Methods: A total of 10 cases of adolescents were treated by first maxillary first molar (group A) or maxillary first premolar (group B). The two groups of samples of gender, age, treatment match each other. Before and after treatment of the two groups of cephalometric films were measured and analyzed to study the different extraction mode before and after treatment of soft and hard tissue changes. All data were SPSS11.5 software package for statistical analysis. Results: U1-L1, S-GO, N-ANS and ANS-Me increased and U1-SN decreased after treatment in both groups. Holdaway soft tissue analysis showed that the upper lip and ditch depth, Distance, H angle, lower lip to H line distance, the depth of the chin groove decreased, the difference between the two groups was not statistically significant. Conclusion: Both of the two methods of tooth extraction can effectively close the incisors and improve the soft tissue appearance. Mandibular extraction location had no significant effect on soft tissue profile change.
