目的了解工人长期持续接触低浓度硫化氢引起的心脏功能改变特征。方法采用定群、纵向研究方法分析研究269名低浓度硫化氢职业接触者2012年在岗期间和2009年岗前的心电图检查结果。结果 2012年心电图异常率(39.03%)高于2009年(26.77%),差异有统计学意义(P<0.01),其中心电轴偏移、肢导低电压、ST-T改变与2009年比较,差异均有统计学意义(均P<0.01);2012年男女心电图异常率均高于2009年,男性异常率差异有统计学意义(P<0.01),而女性异常率差异无统计学意义(P>0.05);2012年心电图异常率46~52岁组>26~35岁组>36~45岁组,46~52岁组与36~45岁组、26~35岁组分别比较,差异均有统计学意义(P<0.01);两个年度间相比,2012年各年龄组异常率均高于2009年,46~52岁组差异有统计学意义(P<0.01)。结论长期持续职业接触低浓度硫化氢和急性硫化氢中毒一样可以导致心脏功能损害,表现出心电图异常,可结合心电图检查结果,判断心肌损伤程度。
Objective To understand the changing characteristics of cardiac function caused by long-term exposure of workers to low concentrations of hydrogen sulfide. Methods The population-based and longitudinal study methods were used to analyze the electrocardiogram results of 269 low-concentration hydrogen sulfide occupational exposure workers in 2012 and before 2009. Results The abnormal rate of electrocardiogram (39.03%) in 2012 was higher than that in 2009 (26.77%), the difference was statistically significant (P <0.01). The changes of central electrical axis, low limb lead voltage, ST- (All P <0.01). The abnormal rates of electrocardiogram in both men and women in 2012 were all higher than those in 2009 (P <0.01), while there was no significant difference in the abnormal rate of female P> 0.05). The abnormal rate of electrocardiogram in 2012 was higher in the group of 46 ~ 52 years old> 26 ~ 35 years old> 36 ~ 45 years old group, 46 ~ 52 years old group and 36 ~ 45 years old group, 26 ~ 35 years old group, (P <0.01). Compared with the two years, the abnormal rates in all age groups in 2012 were higher than those in 2009, and the differences were statistically significant in 46 to 52 years old group (P <0.01). Conclusion Long-term continuous exposure to low concentrations of hydrogen sulfide and acute hydrogen sulfide poisoning can lead to cardiac dysfunction, ECG abnormalities can be combined with the results of electrocardiogram to determine the degree of myocardial damage.
