目的探究磁共振(MRI)对卵巢性索间质肿瘤的诊断价值。方法对我院2012年3月至2013年8月期间收治的40例经手术病例证实卵巢性索间质肿瘤患者的临床、病理以及MRI资料进行回顾性分析;经初步诊断,23例纤维-卵泡膜细胞瘤,5例卵巢纤维瘤,7例卵泡膜细胞瘤,5例卵巢颗粒细胞瘤。40例患者均行MRI平扫及增强扫描。结果 140例卵巢颗粒细胞瘤患者均经MRI证实,正确38例,正确率为95.0%;MRl平扫表现主要包括实性、囊实性、薄膜完整囊性、形态规整、子宫附件区边界清除肿块。214例为囊性肿块,多囊性肿块内见厚薄不一分隔,以较厚为主,囊内壁光滑,囊内容物T_1WI呈等低信号,T_2WI呈高信号;7例为囊实性肿块,T_1WI呈等低混杂信号,T_2WI呈混杂高信号,内有多发大小不等的囊性变.囊内壁光滑;317例为实性肿块,增强扫描10例患者的肿瘤实性部分及囊壁均明显强化。35例卵巢颗粒细胞肿瘤中,1例合并有子宫内膜增厚,3例合并子宫内膜癌,1例合并子宫肌瘤;正确5例(100.0%);47例卵泡膜细胞瘤MRI证实,正确7例(100.0%);55例卵巢纤维瘤MRI证实,正确4例(80.0%);623例纤维-卵泡膜细胞瘤MRI证实,正确22例(95.7%)。结论 MRI诊断卵巢性间质肿瘤的正确率高,值得在临床诊断中推广使用。
Objective To investigate the diagnostic value of magnetic resonance imaging (MRI) in ovarian interstitial stromal tumors. Methods The clinical, pathological and MRI data of 40 patients with ovarian cystic prominence confirmed by surgery in our hospital from March 2012 to August 2013 were analyzed retrospectively. After initial diagnosis, 23 cases of fiber-follicle 5 cases of ovarian fibroma, 7 cases of theca cell tumor and 5 cases of ovarian granulosa cell tumor. All 40 patients underwent MRI plain scan and enhanced scan. Results 140 cases of ovarian granulosa cell tumor were confirmed by MRI, the correct 38 cases, the correct rate was 95.0%; MRl plain manifestations include solid, cystic solidity, complete cystic membrane, . 214 cases of cystic mass, cystic mass see the thickness of different divisions, mainly thicker, smooth capsule wall, capsule content T_1WI was low signal, T_2WI showed high signal; 7 cases of cystic mass, T_1WI was a low mixed signal, T_2WI was a mixed high signal, there are multiple cystic lesions of varying sizes. The wall of the capsule is smooth; 317 cases of solid tumors, enhanced scan in 10 cases of solid tumors and cyst wall were significantly strengthen. Of the 35 ovarian granulosa neoplasms, one had thickening of the endometrium, three had endometrial cancer, and one had uterine fibroids; 5 (100.0%) correctly; Correctly in 7 cases (100.0%); 55 cases of ovarian fibroma MRI confirmed that the correct four cases (80.0%); 623 cases of fibrosa cells of theca cell tumor confirmed that correct in 22 cases (95.7%). Conclusion MRI diagnosis of ovarian stromal tumors with high accuracy, it is worth to promote the use of clinical diagnosis.
