高压氧联合超声波对病理性瘢痕早期干预的疗效观察

目的:探讨高压氧(HBO)联合超声波治疗早期病理性瘢痕的临床疗效。方法:选取2017年6月至2019年11月江门市五邑中医院门诊及住院部收治的病理性瘢痕患者102例作为研究对象,按照随机数字表法分为单纯超声组(超声组)、单纯HBO组(HBO组)以及HBO联合超声波治疗组(联合组),每组34例。超声组采用超声波治疗,HBO组采用HBO治疗,联合组在超声波治疗基础上配合HBO治疗。治疗前后采用温哥华瘢痕量表(VSS)、视觉评估量表(VAS)及患者与观察者瘢痕评估量表(POSAS)对瘢痕愈合情况进行评估。结果:治疗6个疗程后,3组患者VSS、VAS和POSAS总评分均明显低于治疗前(n P<0.05);与超声组和HBO组治疗后比较,联合组VSS、VAS和POSAS总评分明显降低,差异均有统计学意义(n P0.05);HBO组和联合组患者TBSA 2.1%~5.0%的VSS、VAS和POSAS评分差值明显高于TBSA 1.1%~2.0%和0.5%~1.0%患者,差异有统计学意义(n P<0.05)。n 结论:HBO联合超声波早期干预治疗病理学瘢痕具有显著疗效,值得临床推广应用。“,”Objective:To investigate the clinical curative effect of hyperbaric oxygen (HBO) combined with ultrasound on pathological scars in early intervention.Methods:A total of 102 patients with pathologic scars admitted to the outpatient and the inpatient departments of Wuyi Traditional Chinese Medicine Hospital from June 2017 to November 2019 were randomly divided into ultrasound group, HBO group, and HBO+ ultrasound group, with 34 patients in each group. The ultrasound group only received ultrasound treatment; the HBO group only received HBO therapy; the HBO+ ultrasound group received HBO therapy combined with ultrasound treatment. Before and after treatment, Vancouver scar scale (VSS), visual analogue scale (VAS), and the patient and observer scar assessment scale (POSAS) were used to evaluate scar healing.Results:After 6 courses of treatment, the total scores of VSS, VAS, and POSAS in the three groups were significantly lower than those before treatment (n P<0.05). Compared with the ultrasound group and the HBO group after treatment, the total scores of VSS, VAS, and POSAS of the HBO+ ultrasound group were significantly lower (n P0.05). In both the HBO group and the HBO+ ultrasound group, the VSS, VAS, and POSAS scores of the patients with TBSA 2.1%-5.0% were significantly higher than those of the patients with TBSA 1.1%-2.0% and 0.5%-1.0%; and all the differences were statistically significant (n P<0.05).n Conclusion:HBO combined with ultrasound treatment can achieve a significant curative effect on pathological scars in early intervention, which is worthy of clinical application.
