【目的】通过对稳定期慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者应用小剂量茶碱后,肺功能、6min步行试验(six-minute walking distance,6MWD)等指标的检测,探讨小剂量茶碱对COPD患者的预后影响。【方法】应用前瞻、随机分组、对照及自身对照的方法,将1 252例稳定期COPD患者(Ⅱ~Ⅲ级)分为治疗组(茶碱缓释片0.4 g/d)和对照组(安慰剂);对应用茶碱治疗前后的各项指标进行统计学比较。【结果】(1)治疗组的稳定期COPD患者经过75 d的治疗,在用药前后第1 s用力呼气容积实测值与预计值的百分比(FEV1%prep)及FEV1和用力肺活量实测值与预计值的百分比(FEV1/FVC%)较对照组明显升高(P<0.05)。(2)稳定期COPD患者的治疗组与对照组比较,治疗后治疗组的6MWD比对照组增加5.3个月,但结果无统计学意义(P>0.05)。(3)治疗组患者的年内平均感染次数为2.45次/人,对照组年内感染次数为2.99次/人,两者无统计学差异(P>0.05)。【结论】应用小剂量氨茶碱可改善稳定期COPD患者的肺功能,可能会提高患者的运动耐力、降低年内感染次数。

There was no significant difference between the two groups (P> 0.05). 【Conclusion】 The application of low-dose aminophylline can improve lung function in patients with stable COPD, which may improve exercise endurance and reduce the number of infections during the year.
