目的探讨HBV感染型原发性肝癌患者肿瘤微环境中CD_4~+、CD_8~+T细胞及血管内皮生长因子(VEGF)水平及临床意义,为临床治疗提供参考依据。方法前瞻性收集2014年1月-2016年1月收治的HBV感染型原发性肝癌患者57例为研究组,同期收集43例非HBV感染型原发性肝癌患者为对照组;比较两组患者临床指标。结果与对照组比较,研究组患者肿瘤组织微环境中CD_4~+T细胞显著增高、CD_8~+T细胞显著降低;调节性T细胞、VEGF阳性率、肿瘤组织中微血管密度值显著增加,差异均有统计学意义(P<0.05);Pearson线性相关性分析显示,HBV DNA病毒载量与CD_4~+T细胞和调节性T细胞呈正比(P<0.05);与VEGF阴性的患者相比,VEGF阳性HBV感染型原发性肝癌患者微血管密度值计数显著增加。结论 HBV感染型原发性肝癌患者调节性T细胞高表达,可能与HBV增殖有关,且VEGF阳性率增高,与肿瘤血管生成有关。
Objective To investigate the levels of CD_4 ~ +, CD_8 ~ + T cells and vascular endothelial growth factor (VEGF) in the tumor microenvironment of HBV-infected primary hepatocellular carcinoma patients and to provide a reference for clinical treatment. Methods Fifty-seven patients with HBV-infected primary hepatocellular carcinoma (HCC) who were admitted to our hospital from January 2014 to January 2016 were prospectively collected. In the same period, 43 patients with primary hepatocellular carcinoma without HBV infection were recruited as the control group. Clinical indicators. Results Compared with the control group, the CD_4 ~ + T cells in the microenvironment of the study group were significantly increased and the CD_8 ~ + T cells were significantly decreased; the positive rate of regulatory T cells and VEGF, and the microvessel density in the tumor tissues were significantly increased (P <0.05). Pearson linear correlation analysis showed that HBV DNA viral load was positively correlated with CD_4 ~ + T cells and regulatory T cells (P <0.05). Compared with VEGF negative patients, VEGF Microvessel density counts in patients with positive HBV-infected primary hepatocellular carcinoma significantly increased. Conclusion The high expression of regulatory T cells in patients with HBV-infected primary hepatocellular carcinoma may be related to the proliferation of HBV, and the positive rate of VEGF is increased, which is related to tumor angiogenesis.
