目的评价脑转移瘤伽玛刀联合全脑放射治疗的价值。方法将62例脑转移瘤患者随机分为伽玛刀联合全脑放射治疗组(A组,30例)和全脑放射治疗组(B组,32例)。A组先行伽玛刀1次照射治疗,周边剂量为18Gy,中心最大剂量为32~36Gy;再行全脑放射治疗,总剂量为30Gy/10次。B组行全脑放射治疗,总剂量为30Gy/10次,然后转移灶再追加9Gy/3Fx放射治疗。分别对两组患者CT影像学变化及近期疗效进行统计学分析。结果A组头颅CT影像学显示肿瘤体积明显缩小29例(96.7%),临床症状改善明显26例(86.7%),均明显优于B组(P<0.05)。结论伽玛刀联合全脑放射治疗脑转移瘤能提高脑转移瘤的局部控制率。
Objective To evaluate the value of gamma knife radiotherapy for brain metastases. Methods Sixty - two patients with brain metastases were randomly divided into gamma knife combined with whole brain radiotherapy (group A, n = 30) and whole brain radiotherapy (group B, n = 32). Group A first gamma knife treatment, the peripheral dose of 18Gy, the maximum center dose of 32 ~ 36Gy; and then the whole brain radiation therapy, the total dose of 30Gy / 10 times. Group B received whole brain radiation therapy, the total dose of 30Gy / 10 times, and then additional 9Gy / 3Fx metastasis radiation therapy. Respectively, two groups of patients with CT imaging changes and short-term efficacy of statistical analysis. Results The head CT images of group A showed that tumor volume was significantly reduced in 29 cases (96.7%) and clinical symptoms were significantly improved in 26 cases (86.7%), which were significantly better than those in group B (P <0.05). Conclusion Gamma knife combined with whole brain radiation therapy for brain metastases can improve the local control rate of brain metastases.
