[目的]了解江苏省女性乳腺癌死亡率变化及疾病负担,为制定江苏省女性乳腺癌的防控措施提供科学依据。[方法]利用1990~1992年、2003~2005年全死因回顾调查数据以及2013~2015年江苏省死因监测登记数据,比较城乡、不同区域、分年龄组乳腺癌的死亡率和标化死亡率,计算疾病负担指标潜在减寿年数(potential years of life lost,PYLL)、标化潜在减寿年数(standardized potential years of life lost,SPYLL)、潜在减寿率(potential years of life lost rate,PYLLR)、平均减寿年数(average years of life lost,AYLL)。[结果 ]江苏省女性乳腺癌粗死亡率呈逐年上升趋势,1990~2015年累积增长73.21%。同期城市高于农村,苏南和苏中高于苏北,但农村和苏北地区死亡率增长速度更快。女性乳腺癌的死亡率总体随年龄增长而上升,45岁以后快速上升,55岁附近显著增加,80岁以后达到高峰。疾病负担PYLLR呈上升趋势,AYLL近年略有下降。[结论 ]江苏省女性乳腺癌死亡率呈上升趋势;老龄化对乳腺癌死亡影响较大,中老年女性为重点防治对象;应积极开展综合防治措施。
[Objective] To understand the change of breast cancer mortality and the burden of disease in Jiangsu province and to provide a scientific basis for the prevention and control measures of female breast cancer in Jiangsu Province. [Methods] Based on the retrospective survey data of all causes of death from 1990 to 1992 and from 2003 to 2005 and the registration data of the cause of death surveillance in Jiangsu Province from 2013 to 2015, the mortality and standardized mortality of breast cancer in urban and rural areas, different regions and sub-groups were compared. Disease burden indicators were calculated for potential years of life lost (PYLL), standardized potential years of life lost (SPYLL), potential years of life lost rate (PYLLR) Average years of life lost (AYLL). [Result] The crude mortality rate of breast cancer in Jiangsu Province showed an upward trend year by year, with a cumulative increase of 73.21% from 1990 to 2015. Cities in the same period were higher than in rural areas, while those in southern Jiangsu and central Jiangsu were higher than those in northern Jiangsu, but mortality rates in rural and northern Jiangsu grew even faster. Female breast cancer mortality overall increased with age, rapidly rising after 45 years of age, significantly increased near 55 years old, reached the peak after 80 years of age. The burden of disease PYLLR is on the rise, AYLL slightly decreased in recent years. [Conclusion] The mortality rate of breast cancer in Jiangsu Province is on the rise. Aging affects breast cancer death rate to a large extent, and middle-aged and elderly women are the key prevention and treatment targets. Comprehensive prevention and treatment measures should be actively carried out.
