目的:观察白眉蛇毒血凝酶对慢性泪囊炎患者鼻腔泪囊吻合术的有效性和安全性。方法:116例(127眼)慢性泪囊炎患者随机分为试验组与对照组。对照组55例(60眼)行常规鼻腔泪囊吻合术;试验组61例(67眼)于常规鼻腔泪囊吻合术前30 min给予白眉蛇毒血凝酶2 u im。比较两组患者术中出血量,术后鼻腔渗血率、凝血功能和泪道通畅效果,记录药品不良反应。结果:试验组术中总失血量、术后1 d鼻渗血率等指标均明显低于对照组(P<0.05)。两组泪道通畅效果无明显差异;术前术后两组凝血功能指标无明显变化,治疗期间均未观察到明显不良反应。结论:白眉蛇毒血凝酶用于慢性泪囊炎患者鼻腔泪囊吻合术,可以显著降低术中出血量及术后鼻腔渗血率,安全有效。 Objective: To observe the effectiveness and safety of white eyebrow venom hemagglutinin on nasal dacryocystorhinostomy in patients with chronic dacryocystitis. Methods: 116 patients (127 eyes) with chronic dacryocystitis were randomly divided into experimental group and control group. In control group, 55 patients (60 eyes) underwent conventional nasal dacryocystorhinostomy. Sixty-one eyes (67 eyes) in the experimental group were treated with venomous hemagglutinin for 2 minutes before routine dacryocystorhinostomy. The bleeding volume, postoperative nasal bleeding rate, coagulation function and lacrimal patency were compared between the two groups, and adverse drug reactions were recorded. Results: The total amount of blood loss in the experimental group and the rate of nasal bleeding on postoperative day 1 were significantly lower than those in the control group (P <0.05). There was no significant difference between the two groups in the patency of lacrimal duct. There was no significant change in coagulation function between the two groups before operation and no significant adverse reactions were observed during the treatment. Conclusion: The white-browed snake venom hemagglutinin is used in the nasal dacryocystorhinostomy in patients with chronic dacryocystitis, which can significantly reduce the intraoperative blood loss and postoperative nasal bleeding rate, which is safe and effective.
