目的:观察蝎鳖蛎甲汤联合舒尼替尼治疗肾透明细胞癌的临床疗效。方法:选择河北大学附属医院及河北安新县医院2012年3月~2015年3月收治75例肾透明细胞癌患者作为研究对象,随机数字表法分为观察组(n=38)与对照组(n=37)。对照组应用舒尼替尼进行治疗,观察组在对照组基础上加用蝎鳖蛎甲汤,比较两组用药周期、治疗后肿瘤直径、中位无进展生存时间、客观有效率、不良反应发生率。结果:观察组用药周期与中位无进展生存时间均长于对照组,肿瘤直径小于对照组,差异有统计学意义(P<0.05)。观察组客观有效率为76.3%(29/38),明显高于对照组54.1%(20/37),差异有统计学意义(P<0.05)。两组不良反应均以皮肤发黄、甲状腺功能减低、乏力、消化道出血及血小板减少等为主,且观察组皮肤发黄、乏力、消化道出血等的发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:蝎鳖蛎甲汤联合舒尼替尼治疗肾透明细胞癌可有效发挥滋阴潜阳、攻坚破积、清热化毒之功,杀灭肿瘤细胞同时也提高机体免疫功能,减轻舒尼替尼不良反应,促进用药周期与中位无进展生存时间的延长,临床价值显著,值得借鉴、推广。
Objective: To observe the clinical efficacy of Scorpio and Turtle Oyster A decoction combined with sunitinib in the treatment of clear cell renal cell carcinoma. Methods: Seventy-five patients with clear cell renal cell carcinoma were treated in Affiliated Hospital of Hebei University and Anxin Hospital of Hebei Province from March 2012 to March 2015. The random number table was divided into observation group (n = 38) and control group (n = 37). The control group was treated with sunitinib. The observation group was treated with Scorpion and Turtle Oyster Soup Decoction on the basis of the control group. The treatment groups were divided into two groups: treatment group, tumor diameter, median progression-free survival time, objective efficiency and adverse reactions rate. Results: The duration of medication and median progression-free survival in the observation group were longer than those in the control group, and the diameter of the tumor was smaller than that in the control group. The difference was statistically significant (P <0.05). The objective effective rate in the observation group was 76.3% (29/38), which was significantly higher than that in the control group (54.1%, 20/37). The difference was statistically significant (P <0.05). The two groups of adverse reactions were yellow skin, hypothyroidism, fatigue, gastrointestinal bleeding and thrombocytopenia, and the observation group skin yellow, fatigue, gastrointestinal bleeding were significantly lower than the incidence of the control group, the difference was Statistical significance (P <0.05). Conclusion: Scorpion and soft-shelled turtle ointment combined with sunitinib treatment of clear cell renal cell carcinoma can effectively play the role of nourishing yin and yang, tackling firm break, clearing away heat and poisoning the power of killing tumor cells, and also improve immune function and reduce the sunitinib Adverse reactions of nystagmus, promotion of medication cycle and median progression-free survival time are prolonged. The clinical value is significant and it is worth learning from and promoting.
